摘  要:目的探讨高压氧(HBO)在缓解大鼠心肌梗死(AMI)过程中对机体炎症反应和心肌细胞凋亡的调控作用及其机制.方法4周龄的SPF级雄性SD大鼠30只按照数字表法随机分成3组:假手术组、模型组和HBO组.采用冠状动脉结扎法构建大鼠AMI模型,通过超声心动图和血流动力学检测大鼠左心室射血分数(EF)、短轴收缩率(FS)及左室内压上升/下降段最大变化速率(±dp/dtmax)的变化.采用ELISA、qRT-PCR实验检测各组大鼠血清及组织内炎症因子的表达水平.采用Western blotting检测各组大鼠心肌组织中细胞凋亡相关蛋白的表达水平.结果与假手术组比较,模型组的EF和FS显著下降,±dp/dtmax绝对值显著下调(P<0.01);HBO处理后,HBO组大鼠EF、FS和±dp/dtmax均明显高于模型组(P<0.05);模型组大鼠血清炎性因子IL-1β、IL-6、TNF-α和hs-CRP的表达水平显著上调(P<0.01);HBO处理后,HBO组大鼠血清炎性因子IL-1β、IL-6、TNF-α和CRP的表达水平均明显低于模型组(P<0.05).模型组大鼠心肌组织中IL-1β、IL-6、TNF-αmRNA表达水平明显高于对照组(P<0.01);相较于模型组,HBO处理后,可显著下调AMI大鼠心肌组织中IL-1β、IL-6、TNF-αmRNA表达水平(P<0.05).与假手术组比较,模型组大鼠体内促凋亡蛋白Bax、c-caspase-3的表达水平上调,抑凋亡蛋白Bcl-2的表达水平下调(均P<0.05);与模型组比较,HBO处理可显著下调AMI大鼠心肌组织中Bax、c-caspase-3的表达水平,上调Bcl-2的表达水平(P<0.05).结论HBO处理可显著改善AMI大鼠模型组织和细胞的炎症反应及心肌细胞的凋亡情况,进而缓解大鼠AMI症状.Objective To investigate the role of hyperbaric oxygen(HBO)in the regulation of inflammatory response and myocardial cell apoptosis in the process of alleviating myocardial infarction in rats.Methods Thirty 4-week SD rats were randomly divided into 3 groups,i.e.the sham surgery group,the model group and the HBO oxygen group.Acute myocardial infarction(AMI)model was established by coronary artery ligation.Left ventricular ejection fraction(EF),short-axis contraction(FS)and changes in maximum systolic and diastolic pressure(±dp/dtmax)were measured by echocardiography and hemodynamics.The expression levels of inflammatory factors in serum and cardiac muscle tissues of each group were detected by ELISA and qRT-PCR.Western blotting was used to detect the expression levels of apoptosis-related proteins in myocardial tissues of each group.Results As compared with those of the sham surgery group,the EF and FS of the model group significantly decreased and the absolute value of±dp/dtmax was significantly down-regulated(P<0.01).Following HBO treatment,the EF,FS and±dp/dtmax were all significantly higher than those of the model group(P<0.05).The expression levels of IL-6,IL-1β,TNF-αand CRP in the serum inflammatory factors of the model group were significantly up-regulated(P<0.05).After HBO treatment,the expression levels of the IL-1β,IL-6,TNF-αand CRP in the serum inflammatory factors of the HBO group were all lower than those of the model group(P<0.05).The expression levels of IL-1β,IL-6,TNF-αmRNA in the myocardial tissue were all significantly higher than those of the control group(P<0.01).Following HBO treatment,the expression levels of IL-1β,IL-6,TNF-αmRNA in the myocardial tissue of the AMI rats were significantly down-regulated,as compared with those of the model group(P<0.05).The expression levels of bax and c-caspase-3 in the model group were up-regulated,as compared with those of the sham surgical group,while the expression level of Bcl-2 was down-regulated(P<0.05).As compared with the model group,HBO t
