输卵管间质部妊娠患者二维及三维阴道超声成像特点及临床意义

摘　　要：目的 分析输卵管间质部妊娠患者二维及三维阴道超声成像特点及临床意义。方法 选取2016年2月-2019年3月该院收治的102例输卵管间质部妊娠患者为研究对象,采用二维及三维阴道超声检查,比较二维超声及三维阴道超声成像特点。结果 二维超声与三维超声检查孕囊型输卵管间质部妊娠在病变最大径线、病变包块外凸、病变位置、周边肌层类型、周边肌层厚度、边界、间质线征及形态等成像图情况上比较,差异无统计学意义(P>0. 05)。二维超声与三维超声检查混合型输卵管间质部妊娠在病变最大径线和病变包块外凸形态等成像图情况上比较,差异无统计学意义(P>0. 05);而在周边肌层类型、周边肌层厚度、边界及间质线征等成像图情况上比较,差异有统计学意义(P<0. 05)。结论 二维阴道超声成像仅能显示纵切面和横切面,在检查孕囊型输卵管间质部妊娠时具有良好的诊断价值但对混合型输卵管间质部妊娠诊断存在不足,三维阴道超声成像可取得子宫冠状切面图像,可有效诊断孕囊型、混合型两种输卵管间质部妊娠。Objective To analyze the imaging features and clinical significance of two-dimensional and three-dimensional vaginal ul-trasound in patients with interstitial tubal pregnancy.Methods A total of 102 patients with interstitial tubal pregnancy admitted to Xiamen Municipal Maternal and Child Health Care Hospital from February 2016 to March 2019 were enrolled.Two-dimensional and three-dimen-sional vaginal ultrasonography were used to compare the their imaging features.Results There was no statistically significant difference be-tween 2-dimensional ultrasound and 3-dimensional ultrasound in terms of the maximum diameter of the lesion,the extranucity of the lesion mass,the location of the lesion,the type and boundary of the peripheral musculature,the stromal line and the shape of the pregnancy in the cystic tubal interstitium(P>0.05).There was no significant difference in the maximum diameter of the lesion and the protruding shape of the lesion mass in the mixed tubal interstitial pregnancy detected by two-dimensional ultrasound and three-dimensional ultrasound(P>0.05).However,there were statistically significant differences in the types of peripheral muscle layer,peripheral muscle layer thickness,boundary and interstitial lineaments(P<0.05).Conclusion Two-dimensional vaginal ultrasound imaging can only show longitudinal and transverse sections,whichhas a good diagnostic value in gestational sac type interstitial tubal pregnancy but insufficient in mixed interstitial tubal pregnancy.Three-dimensional vaginal ultrasound imaging can obtain images of the coronal section of the uterus,which can effectively diagnose the gestational cyst and mixed types of interstitial tubal pregnancy.
