摘 要:目的探讨聚醚砜膜(PES)与聚砜膜(PS)高通量透析器对炎症因子高敏C反应蛋白(hs-CRP)、β微球蛋白(β2-MG)、甲状旁腺激素(iPTH)的清除效果比较.方法选取2017年2月至2018年2月在本院行血液透析患者60例,依据随机数字表法分为PES组和PS组,每组30例,PS组给予PS高通量透析器治疗,PES组给予PES高通量透析器治疗,比较两组溶质[血磷(SP)、尿素氮(BUN)、血肌酐(Scr)、hs-CRP、β2-MG、iPTH]清除效果及生存情况.结果PES组和PS组治疗6、12个月后SP、BUN、Scr、hs-CRP、β2-MG、iPTH明显低于治疗前,PES组治疗6、12个月后hs-CRP、β2-MG、iPTH明显低于PS组,差异有统计学意义(P<0.05);PES组和PS组治疗6、12个月后SP、BUN、Scr比较,差异无统计学意义(P>0.05);PES组病死率明显的低于PS组,差异有统计学意义(P<0.05).结论与PS高通量透析器治疗比较,PES高通量透析器也可有效清除血液透析患者SP、BUN、Scr,并可有效提高对hs-CRP、β2-MG、iPTH的清除效果,有利于改善患者生存预后,值得临床推广.Objective To discuss the comparison of scavenging effects of polyethersulfone membrane(PES)and polysulfone membrane(PS)high-throughput dialyzers on the inflammatory factor high sensitive C-reactive protein(hs-CRP),beta-microglobulin(beta-2-MG),parathyroid hormone(iPTH).Methods From February 2017 to February 2018,60 hemodialysis patients were selected in our hospital,according to the random number table method,they were divided into PES group and PS group,30 cases in each group.PS group was given PS high-flux dialyzer treatment,PES group was given PES high-flux dialyzer treatment.The scavenging effects of solute[serum phosphorus(SP),urea nitrogen(BUN),serum creatinine(Scr),hs-CRP,beta-2-MG,iPTH]and survival rate were compared between the two groups.Results The 6 and 12 months after treatment,SP,BUN,Scr,hs-CRP,beta 2-MG,iPTH in the PES group and PS group were significantly lower than those in the before treatment,the 6 and 12 months after treatment hs-CRP,beta 2-MG,iPTH in the PES group were significantly lower than those in the before treatme nt,lhe difference was statistically significant(P<0.05).There was not statistically significant difference in SP,BUN,Scr between the PES group and PS group after 6 and 12 months(P>0.05).The mortality rate in the PES group was significantly higher than those in the before treatment,the difference was statistically significant(P<0.05).Conclusions Compared with PS,PES high-throughput dialyzer can also effectively remove SP,BUN and Scr in hemodialysis patients,and it can effectively improve the clearance of hs-CRP,beta-2-MG and iPTH,which is conducive to improving the survival and prognosis of patients,it is worth for further clinical promotion.
