结构变动度视角下ICU患者住院费用变化趋势分析

摘　　要：目的了解山东省某市三级甲等医院ICU患者的住院费用结构变化趋势,分析影响结构变动的主要费用项目和变动情况。方法运用结构变动度、结构变动值和结构变动贡献率等指标,分析ICU患者5年住院费用结构的变动情况。结果5年来ICU患者住院费用结构变动度为21.40%,年均结构变动度为5.35%。引起住院费用变动的主要费用是药品费,其次为治疗费和化验费,三者累积贡献率为69.81%。结论采用结构变动度分析住院费用结构变动情况,可以反映出患者住院医疗费用结构变化的总体特征,应进一步调整药品所占比例,控制材料费、化验费和检查费,提高体现医护人员劳动价值的费用比例。Objective To understand the trend of hospitalization cost structure of ICU patients in Level of first-class hospital of Shandong Province,and identify major expense items and changes that affect structural change.Methods Changes in the structure of hospitalization expenses for five years of ICU patients are analyzed by using structural variability,structural variable value and contribution rate of structural change.Results In the past five years,the structural change rate of hospitalization expenses of ICU patients was 21.40%,and the annual average structural change was 5.35%.The major cost of hospitalization expenses change were drug fees,followed by treatment fees and laboratory fees,and the cumulative contribution rate of the three was 69.81%.Conclusion The structural change of hospital expenditure in ICU department during the past five years was analyzed by structural variability,and the overall characteristics of inpatient medical expenditure structure change could be reflected.The proportion of medicines should be further adjusted,and materials expenses,laboratory fees and inspection fees should be further controlled so as to improve the proportion of the labor value of medical staff.
