Ⅰ类切口手术围手术期预防使用抗菌药物合理性分析

摘　　要：目的分析四种Ⅰ类切口(大隐静脉曲张、骨科取内固定、白内障、腹股沟疝)手术围手术期预防使用用抗菌药物合理性,为临床合理使用抗生素提供依据。方法选取医院2018年1-12月收治的Ⅰ类切口手术患者92例临床资料,对其临床预防应用抗生素进行分析。结果抽取92例Ⅰ类切口手术病例中,预防使用抗生素29例,使用率31. 52%;29例预防性使用抗生素,其中用药时机及疗程合理21例,占72. 41%;29例预防性使用抗生素在用药指征、品种选择、用药时机、用药疗程、用法用量合理率分别为79. 31%、82. 76%、93. 10%、89. 66%、89. 66%;其中联合用药16例,合理13例,合理率81. 25%。结论医院多数Ⅰ类切口能严格遵守抗菌药物使用原则。Ⅰ类切口预防性使用抗菌药物应严格按照抗菌药物临床应用指导原则,选择合适的给药时机、用药疗程,药剂科采用相应的干预措施,加强监督与指导,规范临床用药,科学、预防性使用抗菌药物。Objective To analyze the rationality of the four types of incision( large saphenous varicose veins,orthopedic internal fixation,cataract,inguinal hernia) in the perioperative period to prevent the use of antibiotics,and to provide a basis for clinical rational use of antibiotics. Methods The clinical data of 92 patients with type Ⅰ incision surgery admitted to the hospital from January to December in 2018 were selected,and antibiotics were analyzed for clinical prevention. Results In 92 cases of type Ⅰ incision surgery,29 cases of antibiotics were used,the use rate was 31. 52%;29 cases of preventive use of antibiotics,of which 21 cases were reasonable and the treatment was reasonable,accounting for 72. 41%;29 cases of preventive use of antibiotics in medication indications,variety selection,timing of medication,medication course,usage and dosage rational rate were 79. 31%,82. 76%,93. 10%,89. 66%,89. 66%;Among them,16 cases were combined and 13 cases were reasonable,with a reasonable rate of 81. 25%. Conclusion Most Class Ⅰ incisions in hospitals can strictly follow the principles of antimicrobial use. Prophylactic use of antibacterial drugs for Class Ⅰ incisions should be strictly in accordance with the guidelines for the clinical application of antibacterial drugs,select the appropriate timing of drug administration,medication,and appropriate interventions for the pharmacy,strengthen supervision and guidance,standardize clinical medication,scientific and preventive use.
