腰椎退变患者盘黄间隙与受压神经根DTI相关性

摘　　要：目的探讨旁中央型腰椎间盘突出症患者受压神经根DTI参数FA值与患侧盘黄间隙(DFLS)大小的相关性。方法通过临床体征、查体及腰椎1.5T MRI常规扫描诊断为单侧L5神经压迫的旁中央型椎间盘突出患者18例,行3.0T MRI腰椎DTI及3D-FIESTA-C序列扫描,测量患侧盘黄间隙大小,同时测量健、患侧神经根FA值,计算健、患两侧FA值差值,分析两侧FA值差值与患侧盘黄间隙大小相关性。结果①患、健侧神经根FA值分别为0.260±0.032,0.321±0.042(t=11.055,P<0.01),差异有统计学意义。②患侧盘黄间隙为(1.639±0.539)mm,患侧盘黄间隙与FA值差值呈负相关(r=-0.795,P<0.01)。结论 DTI可定量反映腰神经根损伤程度,盘黄间隙应该成为腰椎退变患者外科术前要考虑的问题。Objective To investigate the correlation between the intervertibral disk-flavum ligamentum space(DFLS) and FA values of compressed nerve root on diffusion tensor imaging(DTI) in patients with paracentral disc herniation.Methods Eighteen patients confirmed with single L5 nerve root compression by clinical examinations and 1.5 T MRI scanning were enrolled.DFLS values of symptomatic side were obtained by 3 D-FIESTA-C on 3.0 T MRI and FA values of L5 nerve roots on both sides were obtained by DTI.The FA values of bilateral nerve roots were measured and the difference of FA values on both sides were calculated in order to discover the correlations between the FA value difference and DFLS.Results ①The mean FA values of bilateral nerve roots were 0.260±0.032,0.320±0.042 respectively and the difference between the two sides was statistically significant(t=11.055,P<0.01).②The mean value of DFLS of symptomatic side was(1.639±0.539)mm.Negative correlation was found between the FA value difference and DFLS values of the compressed nerve roots(r=-0.795,P<0.01).Conclusion DTI parameters are helpful to evaluate the degree of lumbosacral nerve root compression quantitatively.The intervertibral disk-flavum ligamentum space should be considered before surgery.
