摘  要:目的探讨胰岛素泵(CSII)、胰岛素多次皮下注射(MSII)及二甲双胍联合瑞格列奈口服降糖药物3种不同强化降糖方案对初诊2型糖尿病患者体内8-异前列腺素F2α(8-iso-PGF2α)、丙二醛(MDA)的影响。方法初诊2型糖尿病患者123例随机分为胰岛素泵(CSII)组、胰岛素多次皮下注射(MSII)组、二甲双胍联合口服降糖药物组3组,比较治疗前后3组患者体内8-iso-PGF2α、MDA指标变化。结果 3种不同强化降糖方案治疗后血清8-iso-PGF2α、MDA较治疗前均明显降低(P<0.01)。CSII组8-iso-PGF2α、MDA明显低于MSII组和口服降糖药物组(P<0.01),MSII组8-iso-PGF2α、MDA明显低于口服降糖药物组(P<0.05)。多元回归分析显示,年龄和HOMA-IR是影响糖尿病患者8-iso-PGF2α、MDA水平高低的主要影响因素。结论 T2DM患者的8-iso-PGF2α、MDA与年龄和HOMA-IR有关;3组强化治疗均有效降低患者体内的氧化应激和胰岛素抵抗状态,而CSII强化治疗具有更好的抗氧化应激、减轻胰岛素抵抗作用。Objective To elevate the effects of insulin pump(CSII),multiple subcutaneous insulin injection(MSII) and metformin combined with repaglinide on 8-iso prostaglandin F2 alpha(8-iso-PGF2α) and malondialdehyde(MDA) in patients with type 2 diabetes in the first diagnosis.Methods One hundred and twenty three patients with type 2 diabetes were randomly divided into 3 groups,including insulin pump(CSII),insulin multiple subcutaneous injection(MSII),metformin combined with oral hypoglycemic drugs.The changes of 8-iso-PGF2α,MDA,blood glucose and insulin were compared in the three groups of patients before and after treatment.Results Serum levels of 8-iso-PGF2α and MDA were significantly reduced after treatment with three different intensification hypoglycemic regiments(P<0.01).8-iso-PGF2α alpha and MDA in CSII group were significantly lower than MSII and oral hypoglycemic drug group(P<0.01),while 8-iso-PGF2α and MDA in MSII group were significantly lower than oral hypoglycemic drug group(P<0.05).Multiple regression analysis showed that age and HOMA-IR were the main factors affecting the levels of 8-iso-PGF2α and MDA in diabetic patients.Conclusion It proved that 8-iso-PGF2α and MDA in T2 DM patients are related to age and HOMA-IR and the intensive treatments in these three groups can effectively reduce oxidative stress and insulin resistance in patients with T2 DM,while CSII intensive therapy had better anti-oxidative stress and less insulin resistance.
