摘 要:目的探讨妊娠期糖尿病(GDM)孕妇血清及胎盘内中性粒细胞明胶相关脂质运载蛋白(NGAL)及其受体(NGALR)的表达情况及其与胰岛素抵抗的关系。方法选取门诊行常规体检并入院行择期剖宫产的妊娠期糖尿病孕妇35例和正常对照组孕妇32例。检测两组孕妇血清中NGAL、空腹胰岛素(FINS)及空腹血糖(FPG)水平,并计算稳态模型的胰岛素抵抗指数(HOMA-IR)。检测两组孕妇胎盘组织中NGAL和NGALRmRNA及其蛋白的表达水平,并分析其检测指标与胰岛素抵抗的相关关系。结果①GDM组孕妇血清中NGAL,FINS,FPG及HOMA-IR指数均高于正常组孕妇(P<0.05)。②GDM组孕妇胎盘组织中NGALmRNA及蛋白的表达明显高于正常组孕妇(P<0.05),而两组孕妇NGALRmRNA及蛋白的表达无显著差异(P>0.05)。③GDM组孕妇血清中NGAL水平与HOMA-IR呈正相关关系(r=0.542,P<0.05)。④胎盘组织中NGALmRNA和蛋白表达水平与HOMA-IR呈正相关关系(r=0.398,0.436,P<0.05);胎盘组织中NGALRmRNA和蛋白表达水平与HOMA-IR无明显相关性(r=0.119,P>0.05)。结论 NGAL可能通过影响胰岛素抵抗,从而参与GDM的发生。Objective To investigate the expression of neutrophil-associated lipocalin(NGAL) and its receptors in serum and placenta of pregnant women with gestational diabetes mellitus(GDM),and discover its relationship in insulin resistance.Methods Thirty-five pregnant women with GDM and 32 healthy pregnant women were enrolled and divided into GDM group and control group.Serum levels of NGAL,fasting insulin(FINS) and fasting plasma glucose(FPG) were measured in both groups,and the insulin resistance index(HOMA-IR) of the steady state model was calculated.The expression levels of NGAL and NGALR mRNA and protein in the placenta of the two groups were detected,and the correlation between the detection index and insulin resistance was analyzed.Results The serum levels of NGAL,FINS,FPG and HOMA-IR in GDM group were significantly higher than the control group(P<0.05).The expression of NGAL mRNA and protein in placenta in GDM group was significantly higher than the control group(P<0.05),but no significant difference was found in the expression of NGALR mRNA and protein between the two groups(P>0.05).The serum NGAL level in GDM group was positively correlated with HOMA-IR(r=0.542,P<0.05).The expression of NGAL mRNA and protein in placenta tissues was positively correlated with HOMA-IR(r=0.398,0.436,P<0.05).There was no significant correlation between NGALAR mRNA and protein expression levels in placental tissues and HOMA-IR(r=0.119,P>0.05).Conclusion NGAL may participate in the occurrence of GDM by affecting insulin resistance.
