启东市中医院胃肠外科肠外营养使用合理性分析

摘　　要：目的分析启东市中医院胃肠外科肠外营养(PN)使用合理性。方法通过医院信息系统调取2017年2-11月于启东市中医院胃肠外科行胃肠手术的200例住院患者的围术期使用PN制剂、PN开始和结束时间、营养评估、禁食时间等相关信息,根据相关指南和专家共识对其使用合理性进行分析。结果 200例患者中营养风险评估基本缺失,术前禁食时间偏长;PN适应证把握不明确124例,占62%;PN制剂使用不合理(PN适应证不合理和PN疗程不合理)144例,占72%。结论本院胃肠外科PN应用中普遍存在术前禁食时间偏长、营养风险评估基本缺失、PN适应证把握不明确及疗程不合理等问题,合理的个体化方案制定水平与指南要求有一定差距。Objective To analyze the rationality of PN in Gastrointestinal Surgery of Qidong Hospital of TCM.Methods Through the hospital information system,the information about PN preparation,PN start and end time,nutrition evaluation,fasting time and other relevant information of 200 inpatients who underwent gastrointestinal surgery in Qidong Hospital of TCM from February to November 2017 was retrieved,and the rationality of its use was analyzed according to relevant guidelines and expert consensus.Results Among the 200 patients,nutritional risk assessment was basically absent and fasting time was too long before operation;124 patients had unclear PN indications,accounting for 62%;144 patients had unreasonable use of PN preparation(unreasonable PN indications and course of PN treatment),accounting for 72%.Conclusion There are many problems in the application of PN in gastrointestinal surgery in our hospital,such as long fasting time before operation,lack of nutritional risk assessment,unclear indication of PN and unreasonable course of treatment.There is a certain gap between the level of reasonable individualized program formulation and the requirements of guidelines.
