帕累托图和鱼骨图分析法在麻醉药品处方点评中的应用

摘　　要：目的分析帕累托图和鱼骨图分析法在麻醉药品处方点评中的应用价值。方法抽取2018年1-12月沈阳市第四人民医院麻醉药品处方6000张,对其中不合理处方进行帕累托图分析,明确不合理处方类型,进一步运用鱼骨图进行根因分析,提出整改意见。结果不合理处方223张(占3.72%),总不合理用药253例次。帕累托图分析结果显示不合理处方类型中主要因素包括处方书写不规范或缺项、诊断不全、用法用量不适宜、超量;次要因素为给药途径不适宜、选药不适宜;一般因素为超说明说用药、联合用药不适宜、特殊人群用药不适宜。结论利用帕累托图和鱼骨图分析麻醉药品不合理处方类型并进行干预,可促进麻醉药品合理使用。Objective To analyze application value of pareto chart and fishbone diagram analysis in the evaluation of narcotic drug prescriptions. Methods A total of 6 000 prescriptions of narcotic drugs in Shenyang Fourth People’s Hospital from January to December 2018 were selected. Pareto chart was used to analyze the irrational prescriptions, and the types of irrational prescriptions were defined. Fishbone diagram was used to analyze the types of irrational prescriptions, and suggestions for improvement by fishbone diagram were put forward. Results A total of 223 irrational prescriptions(3.72%) were found, with a total of 253 case-times of irrational drug use. Pareto chart analysis showed that the primary factors of irrational prescription types were irregular prescription writing or lack of items, incomplete clinical diagnosis, inappropriate usage and dosage, and exceeding the prescribed dosage;secondary factors included inappropriate route of administration and inappropriate choice of drugs;general factors were off-label drug use, inappropriate combination of drugs, inappropriate use of drugs in special population. Conclusion The use of Pareto chart and Fishbone diagrams to analyze the types of unreasonable prescriptions for anesthesia and intervention can promote the rational use of narcotic drugs can promote the rational use of narcotic drugs.
