摘 要:目的:初步探讨混合现实技术在股骨粗隆间骨折手术中的应用价值。方法:选取1例股骨粗隆间骨折老年女性,术前行薄层CT平扫,获取DICOM格式的二维CT原始影像资料,输入Mimics 20.0软件进行皮肤三维重建、骨折三维重建、制作穿刺导向杆模型、骨折虚拟复位,将重建完毕的皮肤模型、模拟骨折复位的模型、穿刺导向杆模型以STL格式进行输出保存。再将STL格式的模型输入到MR软件中进行混合现实技术显示,用于术前医患沟通、术前讨论手术方案,术中与真正的人体结构进行配准叠加、指导手术操作。结果:在CT原始二维图像的基础上,混合现实技术可以高清显示重建的皮肤模型、骨折模型、穿刺导向杆模型,方便术前医患之间沟通交流,使手术路径清晰、简化手术步骤、减少术中透视次数、缩短手术时间。结论:混合现实技术可真实地呈现出骨折的特点,便于医患交流,提高手术效率,对骨科医疗方式向个体化、精确化、微创化发展有积极促进作用。Objective: To assess the application value of mixed reality technology in intertrochanteric fracture of femur operation. Methods: The CT scanning of 1 patients suffered from intertrochanteric fracture of femur were conduct two-dimensional original image output in DICOM format, input Mimics 20.0 software for skin three-dimensional reconstruction, three-dimensional fracture reconstruction, making puncture guide rod model, and virtual fracture reduction, the reconstructed skin model, simulated fracture reduction model and guide bar model were output and stored in STL format. Then, the models in STL format were input into the MR software for mixed reality display, the MR technology was applied in preoperative communication and discussion, and intraoperative registration overlapped with the actual body structure to guide surgery. Results: On the basis of the original two-dimensional CT images, the reconstructed skin model, fracture model and puncture guide rod model can be displayed in high definition by mixed reality technology, MR technology facilitates communication between doctors and patients before operation, makes the operation path clear, simplifies the operation procedure, reduces radiation during operation, and shortens the operation time. Conclusion: Mixed reality technology can truly show the characteristics of fracture, facilitate the communication between doctors and patients, improve the efficiency of surgery, and promote the development of orthopedic medical treatment to individualization, precision and minimally invasive.
