秦国政从滋肝肾健脾胃散瘀结论治青少年男性乳房发育症

摘　　要：秦国政教授认为青少年男性乳房发育症多因肝肾亏虚、冲任失调、肝失疏泄、脾失健运、胃失和降、痰瘀之邪内蕴日久结于乳络所致。青少年天癸渐至,若禀赋不足,肾之气化失司,冲任不调,水不涵木而横逆脾胃,致痰瘀相搏,结于乳络,发为本病。因此,治疗应顺其发病特点,从病理产物导致的标症入手,兼顾本症。秦教授主张采用滋肝肾健脾胃散瘀结法治疗本病,方选三甲二陈汤加减。滋肝肾常用鳖甲、菟丝子、枸杞子等,健脾胃常用法半夏、陈皮、茯苓等,散瘀结常用夏枯草、穿山甲、老鹳草等。列举验案一则以说明疗效。Professor QIN Guozheng believes that the main causes of juvenile gynecomastia include deficiency of liver and kidney,disharmony of thoroughfare and conception vessels,liver failing to maintain the normal flow of qi,spleen failing to transport,stomach qi failing to descent and phlegm and blood stasis accumulating in the breast collaterals for a long time. For juveniles,if their body are in unfavorable conditions such as constitutional insufficiency,abnormal qi transformation of the kidney,disharmony of thoroughfare and conception vessels,water failing to moistening wood,liver retraining spleen and stomach when they are gradually get their Tiangui(the substance for the promotion of reproductive function),phlegm and blood stasis will accumulate in the breast collaterals and then lead to gynecomastia. Therefore,according to the characteristics of the disease,the treatment should start from symptoms caused by pathological products,at the same time,symptoms caused by the fundamental pathogenesis should also be considered. Professor QIN advocates treating the disease by methods of nourishing the liver and kidney,strengthening the spleen and stomach,dispersing blood stasis and mass. As for prescriptions,he usually uses modified Sanjia Erchen Decoction. In nourishing the liver and kidney,the commonly used medicines include Biejia(Trionycis Carapax),Tusizi(Cuscutae Semen),Gouqizi(Lycii Fructus),and so on. In strengthening the spleen and stomach,the commonly used medicines include Fabanxia(Pinelliae Rhizoma Praeparatum),Chenpi(Citri Reticulatae Pericarpium),Fuling(Poria),and so on. In dispersing blood stasis and mass,the commonly used medicines include Xiakucao(Prunellae Spica),Chuanshanjia(Manis Squama),Laoguancao(Erodii Herba Geranii Herba). One case was listed to illustrate the curative efficacy.
