摘 要:目的探讨新生儿溶血样本对甲状腺功能检测的作用及优化措施,为新生儿的临床诊断提供理论依据。方法选取2017年1月至2018年1月来本院就诊的121例新生儿的1份溶血样本分成两试管并贴上标签后进行试验,其中试管1号作为溶血前样品安放(标准检验),为观察组,试管2号同样为溶血前样品安放(常规检验),为对照组,比较两组新生儿溶血样本中的TSH、FT3、FT4。结果观察组与对照组溶血前的TSH、FT3、FT4相比差异均无统计学意义(P>0.05),溶血后观察组的TSH、FT3、FT4与对照组相比更高,差异有统计学意义(P<0.05);颈静脉、手背静脉的溶血率分别为69.23%和63.64%,足内踝静脉与头皮静脉的溶血率均低于40.00%。结论从检验结果来看,新生儿溶血样本中的FT3、FT4均受样本检验方式(人工溶血样本,采血过程及溶血标本)的影响,但TSH受上述检验方式的影响则相对较小,同时由于FT3、FT4检验结果的升高对新生儿甲状腺功能降低的临床诊治回产生不同影响,因此在临床检验中需引起重视或采取相应的解决措施进行优化处理。Objective To investigate the effect of neonatal hemolysis samples on biochemical test results and optimization measures,and to provide theoretical basis for clinical diagnosis of neonates.Methods One hemolyzed sample of 121 neonates who came to our hospital from January 2017 to January 2018 was divided into two tubes and labeled,and the test tube 1 was placed as a sample before hemolysis(standard test),as the observation group,the test tube No.2 was also placed before the hemolysis sample(conventional test),as the control group and the TSH,FT3 and FT4 in the hemolysis samples of the two groups were compared.Results There was no significant difference in TSH,FT3 and FT4 between the observation group and the control group before hemolysis(P>0.05),the TSH,FT3 and FT4 of the observation group after hemolysis were higher than those of the control group.The significance of learning(P<0.05).The hemolysis rate of jugular vein and dorsal vein of the hand were 69.23%and 63.64%,respectively,the hemolysis rate of the internal iliac vein and scalp vein was less than 40.00%.Conclusion From the test results,FT3 and FT4 in neonatal hemolysis samples are affected by sample test methods(artificial hemolysis samples,blood collection process and hemolysis specimens),but TSH is relatively less affected by the above test methods.Because the increase of FT3 and FT4 test results have different effects on the clinical diagnosis and treatment of neonatal thyroid function reduction,it is necessary to pay attention to the clinical test or take corresponding measures to optimize the treatment.
