摘　　要：范永升教授治认为干燥综合征合并间质性肺病属中医"燥痹""肺痿"范畴。本病的病因病机为内伤燥热,耗气伤津,久而精血亏虚,甚或阴虚血瘀;同时燥热犯肺,出现"肺热叶焦"而成"肺痿";阴损及阳,本病亦可出现阳虚"肺冷"证候。治疗原则为滋脾润燥宣肺。阴虚燥热宜养阴清燥润肺;气阴耗伤宜益气养阴润肺;阴虚血瘀宜养阴祛瘀润肺;阳气亏虚宜益气温阳敛肺;精血亏虚宜温润滋补。根据本病发展的不同阶段可分为阴虚燥热、肺失宣降证,阴虚血瘀、肺失宣降证,气阴亏虚、肺失宣降证,阳虚气弱、肺失宣降证等四大证型。用药特点为重视滋脾润肺、酌情宣利肺气以及适时化痰祛瘀。并举临床案例1则。Professor FAN Yong-sheng believes that Sj?gren’s syndrome with interstitial lung disease belongs to the dryness bi or lung flaccidity. The etiology and pathogenesis of this disease is that the dryness-heat injuring the qi and fluid, and blood deficiency, or yin deficiency and blood stasis. At the same time, the dryness-heat attacks the lungs, appearing ‘hyperpyrexia of the lung’ to ‘lung flaccidity’. Yin deficiency involving yang, this disease can also appear yang deficiency ‘lung cold’ syndrome. The principle of treatment is to nourish the spleen and moisturizing dryness-syndrome. Yin deficiency dryness-heat is suitable for nourishing yin, clearing and moistening lung. Qi and yin consumption should improve the health of the qi and yin and moisten lung. Yin deficiency and blood stasis are suitable for nourishing yin and dispelling stasis and moistening lung. The deficiency of yang qi is suitable for suppling qi to warm yang of the lung. Deficiency of essence and blood should warm and nourish. According to the different stages of the development of this disease, it can be divided into: yin deficiency and dryness heat, lung losing purification and descending syndrome;yin-deficiency and blood-stasis, lung losing purification and descending syndrome;qi and yin deficiency, lung losing purification and descending syndrome;yang and qi deficiency, lung losing purification and descending syndrome. The characteristics of medication are to pay attention to nourishing the spleen and moistening the lungs, opportunely diffusing lung qi and timely dissipating phlegm and dispelling stasis. A clinical case is also presented.
