摘  要:目的:探讨芪参颗粒对心力衰竭大鼠肾脏血流动力学的影响。方法:将50只清洁级大鼠随机分为模型组(n=40)和假手术组(n=10)。模型组通过左冠状动脉前降支结扎术制备大鼠心肌梗死后心力衰竭模型,假手术组只穿线不结扎,其他步骤与模型组相同。术后第3天行心电图检测,将其中造模成功的30只大鼠随机分模型组、福辛普利钠组和芪参颗粒组,每组10只。各组大鼠术后第4天开始给药,每日早晚各1次,芪参颗粒组给予芪参颗粒9.33g/kg,福辛普利钠组给予福辛普利钠片4.67mg/kg,均以用羧甲基纤维素钠溶解方式灌胃给予相应药物,灌胃体积为10mL/kg,连续灌胃给药60d。给药60d后使用肾脏彩色多普勒测定各组大鼠肾内动脉(肾门部肾主动脉、段动脉、叶间动脉)血流收缩期和舒张期峰速度(PSV,EDV)、阻力指数(RI)和加速度(AC)。结果:给药60d后,与假手术组比较,模型组主肾动脉RI显著增加(P<0.05);与模型组比较,福辛普利钠组主肾动脉RI显著下降(P<0.05)。与假手术组比较,模型组肾脏段动脉AC显著下降(P<0.01);与模型组比较,芪参颗粒组肾脏段动脉AC显著升高(P<0.05),且高于同期福辛普利钠组(P<0.05)。各组肾脏叶间动脉PSV、EDV、RI和AC组间差异均无统计学意义。结论:芪参颗粒能够显著改善大鼠肾脏血流动力学指标。Objective: To study the effects of Qishen Granules on renal hemodynamics in the model rats of heart failure induced by left coronary artery ligation. Methods: Totally 50 male SD rats were randomly divided into model group(n=40) and sham operation group(n=10). The model group were prepared by ligation of the left anterior descending coronary artery, and the rats in sham operation group were handled with the identical procedure but without ligation. Electrocardiogram was performed on the third days after operation, and the 30 rats withsuccessful modeling were randomly divided into three groups: model group, fosinopril sodium group, and Qishen Granules group, with 10 rats in each group. All rats began to give drugs twice a day from the fourth day after operation. The rats in Qishen Granules group were given total Qishen Granules by 9.33 g/kg, and rats in fosinopril sodium group were given fosinopril sodium by 4.67 mg/kg, with both intragastric administration of sodium carboxymethyl cellulose by 10 mL/kg. The rats in sham operation group and model control group were treated with the same volume of sodium carboxymethyl cellulose. All rats were given intragastric administration for 60 days. The systolic and diastolic peak velocity(PSV, EDV), resistance index(RI) and acceleration(AC) of the intrarenal artery(main renal artery of renal hilum, segmental renal artery, and arteriae interlobares renis) in rats were mensurated by color Doppler. Results: After 60 days of administration, compared with the sham operation group, RI of the renal aortic model group was increased significantly(P<0.05). Compared with the model group, RI of the renal aorta fosinopril group was decreased significantly(P<0.05). Compared with the sham operation group, the AC of the renal segmental artery model group was decreased significantly(P<0.01). Compared with the model group and fosinopril sodium group, the AC of the renal segmental artery Qishen Granules was increased significantly(P<0.05).
