摘 要:目的探讨基于磁共振成像(MRI)的膝关节病理性半月板挤压临界值。方法选取2018年11月至2019年7月该院的资料完整且行MRI检查患者87例,评估不同内侧半月板挤压临界值对3种膝关节骨性关节炎(OA)结构特征(X射线能诊断的OA、MRI能诊断的骨髓病变和软骨损伤)的预测性能。结果87例受试者内侧半月板挤压均值为(3.8±2.6)mm,内侧半月板挤压测量一致性为0.70(95%CI:0.60~0.80)。内侧半月板挤压临界值为3mm时,预测3种常见OA结构特征的灵敏度为76.3%~84.4%,特异度为54.8%~57.1%;临界值为4mm时的预测灵敏度为71.2%~80.0%,特异度为76.2%~85.7%。结论4mm可作为病理性半月板挤压的替代临界值。Objective To explore the critical value of knee joint pathological meniscus compression based on MRI.Methods From November 2018 to July 2019,87 patients with complete data and MRI examination were selected.The predictive performances of different medial meniscus extrusion thresholds for three types of knee OA structural features(X-ray diagnostic OA,MRI diagnostic bone marrow lesions and cartilage injury)were evaluated.Results The mean value of medial meniscus extrusion in 87 subjects was(3.8±2.6)mm,and the consistency of medial meniscus extrusion measurements was 0.70(95%CI:0.60-0.80).When the critical value of inner meniscus extrusion was 3 mm,the sensitivity of predicting three common OA structural features was 76.3%-84.4%,and the specificity was 54.8%-57.1%.When the critical value is 4 mm,the sensitivity was 71.2%-80.0%,and the specificity was 76.2%-85.7%.Conclusion 4 mm can be used as an alternative critical value for pathological meniscus extrusion.
