摘 要:目的探讨2型糖尿病(T2DM)合并亚临床甲状腺功能紊乱是否存在血脂水平的变化。方法回顾性分析2012年12月至2015年12月该院收治的605例T2DM患者的临床资料,其中甲状腺功能正常患者308例(甲功正常组),合并亚临床甲状腺功能减退症(甲减)患者205例(亚临床甲减组),合并亚临床甲状腺功能亢进症(甲亢)患者92例(亚临床甲亢组),并根据年龄分为小于30岁组、30~<40岁组、40~<50岁组、50~<60岁组、60~<70岁组、≥70岁组。检测各组患者甲状腺功能、糖化血红蛋白、血脂等指标。结果亚临床甲减组患者血清三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平与甲功正常组比较,差异均无统计学意义(P>0.05)。亚临床甲亢组患者仅血清TC、TG水平明显低于甲功正常组,差异均有统计学意义(P<0.05),而HDL-C、LDL-C水平与甲功正常组比较,差异均无统计学意义(P>0.05)。30~<40岁组中,亚临床甲亢组患者TG水平明显低于甲功正常组患者,差异有统计学意义(P<0.05)。其余各年龄组中,亚临床甲亢组患者TC、TG、HDL-C、LDL-C水平与甲功正常组患者比较,差异均无统计学意义(P>0.05)。结论T2DM合并亚临床甲减与甲状腺功能正常患者比较,未发现TC、TG、HDL-C、LDL-C代谢异常。而T2DM合并亚临床甲亢患者存在低总胆固醇症及低三酰甘油血症。Objective To investigate whether the changes of serum lipid levels existing in type 2 diabetes(T2DM)complicating subclinical thyroid dysfunction.Methods The clinical data in 605 cases of T2DM treated in the hospital from December 2012 to December 2015 were retrospectively analyzed,including 308 cases of normal thyroid function(normal thyroid function group),205 cases of complicating subclinical hypothyroidism(subclinical hypothyroidism group)and 92 cases of complicating subclinical hyperthyroidism(subclinical hyperthyroidism group).The cases were divided into the<30 years old group,30-<40 years old group,40-<50 years old group,50-<60 years old group,60-<70 years old group and≥70 years old group according to the age.The thyroid function,glycated hemoglobin and blood lipids were detected in each group.Results The levels of serum triglycerides(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)had no statistical difference between the subclinical hypothyroidism group and the normal thyroid function group(P>0.05).Only the TC and TG levels in the subclinical hyperthyroidism group were markedly lower than those in the normal thyroid function group(P<0.05),and the HDL-C and LDL-C levels had no statistical difference compared with the normal thyroid function group(P>0.05).In the 30-40-year-old group,the TG level of the subclinical hyperthyroidism group was significantly lower than that of the normal thyroid function group,and the difference was statistically significant(P<0.05).In the other age groups,the TC,TG,HDLC and LDL-C levels of the subclinical subclinical hyperthyroidism group group were not statistically significant compared with those of the normal thyroid function group(P>0.05).Conclusion In the comparison between the patients with T2DM complicating subclinical hypothyroidism and the patients with normal thyroid function,the metabolic abnormality of TC,TG,HDL-C and LDL-C is not found.But the patients with T2DM complicating subclinical hyperthyro
