反转联合双手托颌法用于I-gel喉罩置入的研究

摘　　要：目的:比较标准方法和反转联合双手托颌法用于置入I-gel喉罩的效果。方法:选择择期行支气管镜诊治患者171例,采用随机数字表法分为两组:标准方法组(standard technique group, S组)(85例),采用标准方法置入I-gel喉罩;反转联合双手托颌法组(reverse combined with jaw-thrust technique group, RJT组)(86例),采用反转联合双手托颌法置入I-gel喉罩。记录两组患者喉罩置入成功率、置入时间、术毕拔出喉罩是否有血迹和术后24 h咽痛情况。结果:两种方法总成功率差异无统计学意义。与S组相比,RJT组第1次成功率明显增高,第1次成功置入时间及成功置入总时间明显缩短,且出血及术后24 h咽痛发生率明显降低(P<0.05)。结论:与标准方法相比,采用反转联合双手托颌法置入I-gel喉罩首次成功率更高,耗时更短,损伤更小。Objective : To compare the effect of two different insertion technique with I-gel: standard technique and reverse combined with jaw-thrust technique. Methods : One hundred and seventy-one patients undergoing bronchoscope were randomly divided into two groups. In the standard technique group(group S, 85 cases), the I-gel was inserted using the standard technique. In the reverse combined with jaw-thrust technique group(group RJT, 86 cases), the I-gel was inserted using the reverse combined with jaw-thrust technique. Results : There was no significant difference in the overall success rate between the two methods. The first-attempt insertion success rate in group S was lower than that in group RJT. The insertion time in group S was longer than that in RJT. The incidence of blood staining and sore throat at 24 h postoperatively was higher in group S. Conclusion : Reverse combined with jaw-thrust technique for the placement of I-gel resulted in appropriate placement with easier insertion, lower placement time and less trauma than that with the standard technique.
