摘 要:目的分析2014-2018年芜湖地区丙氨酸氨基转移酶(ALT)临界值2次变更前后血液报废率变化,探讨ALT检测值的高低与HBV、HCV感染的相关性。方法对本单位2014-2018年献血者标本ALT临界值2次变更前后ALT不合格率进行统计分析,并结合302例ALT单项不合格标本的核酸检测结果,探讨ALT与HBV、HCV阳性的相关性。结果男、女ALT报废率差异有统计学意义;单独提高男性临界值至50 U/L和男、女临界值均提高至50 U/L后,ALT报废率分别由3.55%下降至2.27%和1.50%,差异均有统计学意义(P<0.05);HBV阴性组和阳性组ALT不合格率分别为1.51%和1.45%,差异无统计学意义(P>0.05),HCV阴性组和阳性组ALT不合格率分别为1.50%和3.92%,差异有统计学意义(P<0.05);302例ALT单项不合格标本的HBV、HCV核酸检测结果均阴性。结论调高临界值后血液报废率大大降低,为了在血液报废和输血安全这两方面寻找平衡,建议再次提高临界值或单独提高男性临界值。Objective To analyze the change of blood discard rate before and after the change of alanine aminotransferase(ALT) critical value in Wuhu area from 2014 to 2018, and to explore the correlation between ALT, HBV and HCV infection. Methods The statistical analysis of ALT unqualified rate before and after twice changes of ALT critical value of blood donor specimens from 2014 to 2018, combined with the nucleic acid test results of 302 ALT single unqualified specimens, were applied to explore the correlation between ALT,HBV and HCV positive. Results The difference in ALT unqualified rate between men and women was statistically significant. After the male threshold was raised to 50 U/L and both of male and female thresholds to 50 U/L, the ALT unqualified rate decreased from 3.55% to 2.27% and 1.50%,respectively. The difference was statistically significant(P<0.05).The ALT unqualified rates of HBV-negative and positive groups were 1.51% and 1.45%, respectively, and the difference was not statistically significant(P>0.05).The ALT unqualified rates of HCV-negative and positive groups were 1.50% and 3.92%, respectively, and the difference was statistically significant(P<0.05).The results of HBV and HCV nucleic acid test were both negative in 302 cases of ALT single unqualified samples.Conclusion After the critical value is increased, the blood discard rate is greatly reduced. In order to find a balance between blood discard and blood transfusion safety, it is recommended to raise the critical value again or increase the male critical value alone.
