摘　　要：目的检测2型糖尿病早期肾病患者的尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和胱抑素-C的水平,并与无糖尿病肾病的患者进行比较,分析尿NGAL和胱抑素C水平与尿微量白蛋白相关性。方法入组126名2型糖尿病患者和30名无糖尿病肾病作为对照组进行横断面比较研究。分为3个研究组:2型糖尿病有微量白蛋白尿组,2型糖尿病无微量白蛋白尿组和无任何肾脏疾病的患者。统计研究对象中2型糖尿病病程时间和血糖状态。对所有受试者的血样集中储存在-20℃的冰箱中,批量检测尿微量白蛋白以及NGAL和胱抑素-C水平。结果微量白蛋白尿患者的尿NGAL和胱抑素-C水平(228.18和3.23ng/ml)与无蛋白尿(146.12和2.61ng/ml)和对照组(26.56和0.30ng/ml)相比显著升高。尿NGAL和胱抑素C水平与糖尿病患者微量白蛋白尿呈线性相关。结论尿NGAL和胱抑素C水平早期糖尿病肾病患者较无糖尿病肾病患者升高。尿NGAL和胱抑素-C水平与2型糖尿病患者微量白蛋白尿(尿白蛋白肌酐比值)呈正相关。Objective To detect the levels of urinary neutrophil gelatinase-associated lipocalin(NGAL)and cystatin-C in patients with type 2 diabetic early nephropathy and to compare urinary NGAL with patients without diabetic nephropathy.Cystatin C levels correlate with microalbuminuria in urine.Methods A cross-sectional comparative study was conducted in 126 patients with type 2 diabetes and 30 patients without diabetic nephropathy as a control group.Divided into three research groups:type 2diabetes with microalbuminuria,type 2 diabetes without microalbuminuria and patients without any kidney disease.The duration of disease and blood glucose status of type 2 diabetes were statistically studied.Blood samples from all subjects were stored in a freezer at-20℃,and urine microalbumin as well as NGAL and cystatin-C levels were measured in bulk.Results Urinary NGAL and Cystatin-C levels(228.18 and 3.23ng/ml)were compared with no proteinuria(146.12 and 2.61ng/ml)and control(26.56 and 0.30ng/ml)in patients with microalbuminuria.Significantly elevated.Urinary NGAL and Cystatin C levels were linearly associated with microalbuminuria in diabetic patients.Conclusion Urinary NGAL and Cystatin C levels are elevated in patients with early diabetic nephropathy compared with patients without diabetic nephropathy.Urinary NGAL and Cystatin-C levels were positively correlated with microalbuminuria(urinary albumin creatinine ratio)in type 2diabetic patients.
