球后麻醉呼吸抑制综合征致死1例

摘　　要：目的:探讨球后麻醉呼吸抑制综合征致死患者1例。方法:选取球后麻醉呼吸抑制综合征导致死亡患者1例,比较患者右眼与左眼的手术数据,右眼为经手术后正常治愈,左眼为经手术治疗时患者死亡。结果:患者右眼ECG、BP、SPO2、MAP、HR、PETCO2水平均明显优于左眼,差异有统计学意义(P<0.05)。结论:球后麻醉方法虽有变化,但差异不大,风险与操作者经验和眼球结构异常情况以及患者的配合相关,建议球后麻醉由专人或在有经验的医师指导进行,并准备好心电监护、开通静脉通路、呼吸给氧,应在麻醉师在岗手术室侯岗情况下进行,并备好抢救设备和药品,手术医生、护士、麻醉师和内科医生均应熟悉麻醉意外的抢救流程和技术操作,防患于未然。Objective:To explore a fatal case of postbulbar anesthesia with respiratory depression syndrome.Methods:One fatal case of postbulbar anesthesia with respiratory depression syndrome was selected,we compared the surgical data of right eye with the left eye,the right eye was normally cured by surgery,the operative patient of the left eye was dead.Results:ECG,BP and SpO2 levels in the right eye were better than those in the left eye.The level of MAP,HR and PETCO2 of respiratory function index in the right eye were better than those in the left eye,the difference was statistically significant(P<0.05).Conclusion:Although there were some changes in the methods of retrobulbar anesthesia,the differences were not significant,the risk should be related to the operator's experience,abnormal eyeball structure and patient's cooperation,it was suggested that post ball anesthesia should be conducted by specially assigned person or under the guidance of experienced person,meanwhile,we should be prepared for cardiac electrical monitoring,opening of venous access,breathing and oxygen supply,the anesthesiologist was waiting for the post in the operating room,and we should prepare rescue facilities,equipment and drugs,surgeons,nurses,anesthesiologists and physicians should be familiar with the rescue process and technical operation of anesthesia accidents,taking all measures to prevent any risk.
