摘　　要：目的:探究经皮经伤椎椎弓根钉内固定与经皮跨伤椎内固定联合PVP治疗胸腰段单节段椎体骨折的临床效果。方法:2018年9月-2019年9月收治胸腰段单节段椎体骨折患者72例,按照接诊顺序奇偶数分为两组各36例。观察组选择经皮跨伤椎内固定联合PVP治疗,对照组选择经皮经伤椎椎弓根钉内固定治疗。对比两组手术相关指标水平、临床疗效、伤椎恢复及疼痛评分情况。结果:观察组临床治疗总有效率高于对照组,差异有统计学意义(P<0.05)。两组术中出血量比较,差异无统计学意义(P>0.05)。观察组住院时间、手术时间和术后下床时间均短于对照组,差异有统计学意义(P<0.05)。两组治疗后脊柱生理弯曲、疼痛程度、椎体高度均出现不同程度减轻或恢复,但两组术后伤椎恢复情况、疼痛比较,差异无统计学意义(P>0.05)。结论:针对胸腰段单节段椎体骨折患者采用经皮跨伤椎内固定联合PVP治疗,可显著改善患者生活质量,缩短手术时间、术后下床时间、住院时间,加快患者疾病康复速度,具有临床推广价值。Objective:To explore the clinical effect of percutaneous transpedicular screw fixation through the injured vertebral body and percutaneous transpedicular screw fixation crossing the injured vertebral body combined with PVP in the treatment of thoracolumbar single segment vertebral fractures.Methods:From September 2018 to September 2019,72 patients with thoracolumbar single segment vertebral fractures were selected,and they were divided into two groups with 36 cases in each group according to the parity of reception sequence.The observation group was treated with percutaneous transpedicular screw fixation crossing the injured vertebral body combined with PVP,the control group was treated with percutaneous transpedicular screw fixation through the injured vertebral body combined with PVP,we compared the level of operation related indexes,clinical effect,recovery of injured vertebrae and pain score between the two groups.Results:The total effective rate of clinical treatment in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in intraoperative hemorrhage between the two groups(P>0.05).In the observation group,the time of hospitalization,operative time and getting out of bed after operation were shorter than those in the control group,the difference was statistically significant(P<0.05).After treatment,the physiological curvature of spine,the degree of pain and the height of vertebral body in the two groups decreased or recovered in different degrees,however,there was no significant difference between the two groups in the postoperative recovery and pain score of injured vertebrae(P>0.05).Conclusion:The application of percutaneous transpedicular screw fixation crossing the injured vertebral body combined with PVP in the treatment of thoracolumbar single segment vertebral fractures can significantly improve the quality of life of patients,shorten the operation time,postoperative time out of bed,hospital stay,and spe
