五位一体湿化给氧固定装置在气管切开非机械通气患者中的应用

摘　　要：目的探讨五位一体湿化给氧固定装置在气管切开非机械通气患者中的应用效果。方法选取2016年1~12月我院ICU收治的60例气管切开后非机械通气患者作为研究对象,按随机数字表法分为观察组和对照组,每组各30例。湿化液均采用0.9%生理盐水25 ml+灭菌注射用水25 ml,气源均为中心供氧,观察组采用五位一体湿化给氧固定装置,具体方法为微量泵联合三通持续泵入,然后将三通的延长管端固定于自制的墨菲滴管套管内,最后将墨菲套管固定在气管切开套管外口行气道湿化;对照组采用微量泵结合氧气管持续泵入方法。观察两组痰液粘稠度、气道湿化效果、给氧装置脱出次数及不同时间血气分析动脉血氧分压结果。结果观察组痰液粘稠度及湿化效果优于对照组,差异有统计学意义(P<0.05);观察组给氧装置脱出次数少于对照组,观察组不同时间血气分析动脉血氧分压高于对照组,差异有统计学意义(P<0.05)。结论五位一体湿化给氧固定装置用于气管切开非机械通气患者的湿化效果好,可提高患者的动脉血氧分压,减少肺部感染发生情况,并发症少,且取材方便,操作简单,提高患者的临床治疗效果。Objective To explore the application effect of the five-in-one wetoxygen fixation device in patients with tracheotomy in-service non-mechanical ventilation.Methods From January to December 2016,60 patients admitted to the ICU of our hospital after tracheotomy were treated for non-mechanical ventilation as the objects of study,and cases were divided into observation group and control group according to the random numerical table method,30 cases in each group.The wetliquid was divided into 0.9%physiological saline 25 ml plus 25 ml of sterilizing injection water,and the gas source was central oxygen supply,the observation group was adopted the five-in-one humidification oxygen fixation device,the specific method was the micro-pump combined three-way continuous pumping,and then fixed the extension tube end of the tee in the homemade Murphy′s dropper sleeve,and finally fixed the Murphy′s casing in the outer mouth of the tracheotomy cut sleeve for airway moisture.The results of sputum viscosity,airway wetting effect,number of releases of oxygen devices and blood gas analysis of arterial blood fractional results were observed in two groups of patients.Results The viscosity and wetting effect of sputum musalism in the observation group was better than that of the control group,and the differences were statistically significant(P<0.05).The number of releases of oxygen devices in the observation group was fewer than that of the control group,and the blood gas analysis of arterial blood oxygen level at different times in the observation group was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusion The five-in-one wetoxygen fixation device is applied to the wetting effect of patients with tracheotomy non-mechanical ventilation,which can improve the patients′arterial blood oxygen pressure,reduce the occurrence of lung infection,have fewer complications,and be convenient to obtain,simple to operate,and improve the patient′s clinical treatment effect.
