双极调压脉冲射频联合小剂量胶原酶治疗腰椎间盘突出症29例临床研究

摘　　要：目的:探讨双极调压脉冲射频联合小剂量胶原酶治疗腰椎间盘突出症的临床疗效及安全性。方法:选择腰椎间盘突出症患者60例,随机分为双极调压脉冲射频联合小剂量胶原酶溶解术治疗组(A组29例)和胶原酶盘外溶盘术治疗组(B组31例);分别在C臂机引导下完成手术。采用改良Macnab方法评估观察术后1周、1月、3月、6月的疗效,并记录术后不良反应;测量并对比术前与术后3月、6月椎间盘突出指数及CT值。结果:A组患者术后1周、1月、3月和6月优良率明显高于B组,组间相比差异有统计学差异(P<0.05);两组患者术后3月、6月的椎间盘突出指数较治疗前相比差异有统计学差异(P<0.05),组间比较无统计学差异。结论:双极调压脉冲射频联合小剂量胶原酶治疗腰椎间盘突出症较单纯胶原酶盘外溶盘术治疗疗效更好、更安全,能有效避免较大剂量的胶原酶引起的潜在并发症。Objective:To explore the clinical effects and safety of the bipolar needle manual pressure regulator pulsed radiofrequency combined small dose of collagenase on lumbar intervertebral discherniation(LIDH).Methods:60 patients with LIDH were treated with bipolar needle manual pressure regulator pulsed radiofrequency.The patients were followed up at 1 week,1 month,3 month and 6 month after treatment.The clinical effects were evaluated with modified Macnab method and side effects were recorded.The index and CT value of intervertebral disc were measured and compared before and after operation. Results:The excellent and good rates of group A was significantly higher than group B in 1 week,1 month,3 months and 6 months after surgery.In the two groups,the index of intervertebral disc herniation in 3 months and 6 months after surgery was significantly different than before treatment.There was no significant differences between groups.Conclusion:Bipolar needle manual pressure regulator pulsed radiofrequency is more safe and effective minimally invasive treatment for LIDH.
