Foley尿管低位小水囊、米索前列醇和缩宫素三种方式在妊娠晚期引产中应用效果比较

摘　　要：目的比较Foley尿管低位小水囊、米索前列醇和缩宫素用于妊娠晚期引产的效果。方法选择2016年12月—2018年12月在该院收治的妊娠晚期初产妇156名,依据引产方法分为Foley组、米索前列醇组和缩宫素组,每组52名。观察3组引产效果。结果引产后,Foley组产妇宫颈成熟有效率为90.4%,高于米索前列醇组的71.2%和缩宫素组的55.8%,差异有统计学意义(χ^2=20.267、6.190,P<0.05)。缩宫素组产妇干预至临产时间、总产程时间、产后出血量及剖宫产率分别为(14.6±2.9)h、(9.4±2.4)h、(354.4±76.3)mL,均显著大于Foley组和米索前列醇产妇,差异有统计学意义(t=4.538、3.146、4.824、3.328、3.473、2.784,P<0.05)。3组产妇所引产的新生儿Apgar评分比较,差异无统计学意义(P>0.05)。结论Foley尿管低位小水囊用于孕晚期妊娠引产效果更佳,可降低剖宫产率,安全有效。Objective To compare the effect of Foley's low sac small water sac,misoprostol and oxytocin in induction of labor in late pregnancy.Methods A total of 156 gestational gestational women who were admitted to the hospital from December 2016 to December 2018 were enrolled.The patients were divided into Foley group,misoprostol group and oxytocin group according to the induction method,52 cases in each group.Observe the effects of three groups of induction labor.Results After induction of labor,the effective rate of maternal cervical ripening in the Foley group was 90.4%,which was higher than 71.2%in the misoprostol group and 55.8%in the oxytocin groupthe difference was statistically significant(χ^2=20.267,6.190,P<0.05).Maternal intervention in the oxytocin group to the time of labor,total labor,postpartum hemorrhage and cesarean section were(14.6±2.9)h,(9.4±2.4)h,(354.4±76.3)mL,respectively,were significantly greater than the Foley group and women with misoprostol the difference was statistically significant(t=4.538,3.146,4.824,3.328,3.473,2.784,P<0.05).There was no significant difference in the Apgar scores of the neonates induced by the three groups of women,the difference was not statistically significant(P>0.05).Conclusion The Foley urethral small water sac is better for induction of labor in the third trimester of pregnancy,which can reduce the cesarean section rate and is safe and effective.
