重叠综合征患者血清白介素-6水平与肺功能的关系

摘　　要：目的探讨重叠综合征(overlap syndrome,OS)患者的血清白介素?6(interleukin?6,IL?6)水平与病情严重程度及肺功能的关系。方法选取2016年6月至2018年6月本院36例OS患者(OS组)、40例慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者、30名健康人(对照组),检测并比较各组临床资料、多导睡眠监测、肺功能及血清IL?6水平。结果OS组患者的身体质量指数(body mass index,BMI)、颈围、呼吸暂停低通气指数(apnea?hypopnea Index,AHI)显著高于COPD组及对照组,LSaO2、PaO2明显低于COPD组(均P<0.05);OS组FEV1%、FEV1/FVC明显低于COPD组,R20、R5明显高于COPD组(均P<0.05);OS组、COPD组血清IL?6水平明显高于对照组(P<0.01),且OS组高于COPD组(P<0.01);OS组血清IL?6水平与BMI、AHI呈显著正相关(P<0.01),与LSaO2、PaO2、FEV1%及FEV1/FVC呈显著负相关(P<0.01)。结论OS患者血清IL?6升高与睡眠呼吸紊乱、缺氧及肺功能损害有一定的相关性,可作为监测OS患者病情及判断预后的参考指标。Objective To investigate the relationship between serum IL?6 level and severity of disease and lung function in patients with overlapping syndrome(OS).Methods 36 patients with OS(OS group),40 pa?tients with COPD(COPD group)and 30 healthy people(control group)were selected to detect and compare clini?cal data,polysomnography,lung function and serum IL?6 level of each group.Results BMI,neck circumference and AHI of patients in OS group were significantly higher than those in COPD group and control group(P<0.05).LSaO2 and PaO2 were significantly lower than those in COPD group(P<0.05).FEV1%and FEV1/FVC in the OS group were significantly lower than those in the COPD group,and R20 and R5 were significantly higher than those in the COPD group.FEV1%and FEV1/FVC in the OS group were significantly lower than those in the COPD group(P<0.05),serum IL?6 levels in the OS group and COPD group were significantly higher than those in the control group(P<0.01),and serum IL?6 levels in the OS group were significantly higher than those in the COPD group(P<0.01).In OS group,serum IL?6 level was positively correlated with BMI and AHI(P<0.01),and negative?ly correlated with LSaO2,PaO2,FEV1%and FEV1/FVC(P<0.01).Conclusion The increase of serum IL?6 in OS patients is correlated with sleep respiratory disorder,hypoxia and lung function damage to a certain extent,which can be used as a reference index to monitor the condition of OS patients and judge the prognosis.
