灵龟八法针刺治疗卒中后肩手综合征Ⅰ期临床观察

摘　　要：目的验证灵龟八法针刺治疗卒中后肩手综合征Ⅰ期患者的临床疗效。方法将112例脑梗死、脑出血合并Ⅰ期肩手综合征患者随机分成治疗组和对照组,每组56例。对照组采用常规针刺疗法结合康复训练治疗,治疗组在康复训练的基础上加以灵龟八法针刺治疗(根据就诊时辰选择开穴,足临泣-外关/申脉-后溪)。在治疗前后评估两组患者的疼痛评分(VAS)、肢体运动功能(FMA量表)、日常生活能力(Bathel指数)和临床疗效。结果两组患者治疗后VAS评分、FMA评分、Bathel指数评分均较治疗前改善(P<0.05);治疗组治疗后VAS评分低于对照组(P<0.05),Bathel指数评分高于对照组(P<0.05),治疗组疗效优于对照组(P<0.05)。结论灵龟八法针刺疗法可改善卒中后肩手综合征Ⅰ期患者肩关节疼痛、运动功能和日常活动能力。Objective To verify the clinical efficacy of Ling Gui Ba Fa(eight magic turtle acupuncture techniques)in treating patients with post-stroke shoulder-hand syndrome stageⅠ.Method A total of 112 patients with cerebral infarction or hemorrhage coupled with shoulder-hand syndrome stageⅠwere randomized into a treatment group and a control group,with 56 cases in each group.The control group was intervened by ordinary acupuncture method plus rehabilitation training;the treatment group was given acupuncture treatment following Ling Gui Ba Fa[chose the opening acupoint according to the visiting hour,Zulinqi(GB41)-Waiguan(TE5)/Shenmai(BL62)-Houxi(SI3)]plus the rehabilitation training.The Visual Analogue Scale(VAS),limb motor function[Fugl-Meyer Assessment(FMA)]and activities of daily living[Barthel Index(BI)]were estimated before and after treatment.The clinical efficacy was also evaluated.Result The VAS,FMA and BI scores were all improved after treatment in the two groups(P<0.05).The VAS score was lower in the treatment group than in the control group after treatment(P<0.05),and the BI score in the treatment group was higher than that in the control group(P<0.05).The efficacy was more significant in the treatment group than in the control group(P<0.05).Conclusion Acupuncture with Ling Gui Ba Fa can improve the shoulder joint pain,motor function and activities of daily living in patients with post-stroke shoulder-hand syndrome stageⅠ.
