穴位贴敷联合红外线照射治疗糖尿病周围神经病变临床研究

摘　　要：目的探讨穴位贴敷加红外线照射联合个性化护理对糖尿病周围神经病变(DPN)患者疗效及生活质量的影响。方法将150例DPN患者随机分为观察组、安慰组和对照组,每组50例。对照组给予基础治疗和常规护理干预,观察组给予穴位贴敷加红外线照射治疗及个性化护理干预,安慰组给予穴位贴敷安慰剂加红外线照射治疗及个性化护理干预。对比3组治疗前后多伦多临床评分(TCSS)、震动感觉阈值(VPT)检查结果、生活质量简易量表SF-36及治疗总有效率和护理质量。结果3组治疗后TCSS评分、VPT检查结果明显降低(P<0.05),生活质量评分均明显升高(P<0.05)。3组之间比较差异有统计学意义(P<0.05),观察组最优,安慰组次之。观察组和安慰组健康教育知识知晓率较对照组提升更为明显(P<0.05),观察组和安慰组护理满意度明显高于对照组(P<0.05),观察组和安慰组护理满意度比较差异无统计学意义(P>0.05)。结论穴位贴敷加红外线照射联合个性化护理可明显改善DPN患者神经功能和生活质量,提高医疗和护理效果。Objective To investigate the efficacy of acupoint application plus infrared radiation and individualized nursing and its effect on the quality of life in patients with diabetic peripheral neuropathy(DPN).Method One hundred and fifty DPN patients were randomized to observation,placebo and control groups,50 cases each.The control group received basic treatment and conventional nursing;the observation group,acupoint application plus infrared radiation and individualized nursing;the placebo group,acupoint application placebo plus infrared radiation and individualized nursing.The Toronto Clinical Scoring System(TCSS)scores,vibration perception thresholds(VPT),the MOS 36-Item Short-Form Health Survey(SF-36)scores,the total efficacy rates of treatment and nursing qualities were compared between the three groups before and after treatment.Result After treatment,the TCSS score,VPTs decreased significantly in the three groups(P<0.05)and the SF-36 score increased significantly in all three groups(P<0.05).Of which,the observation group showed the most significant improvements,followed by the placebo group,and the inter-group comparisons showed statistical significance(P<0.05).The awareness rate of health education knowledge increased significantly more in the observation and placebo groups compared with the control group(P<0.05).Nursing satisfaction was significantly greater in the observation and placebo groups than in the control group(P<0.05).There was no statistically significant difference in nursing quality between the observation and placebo groups(P>0.05).Conclusion Acupoint application plus infrared radiation and individualized nursing can markedly improve neural functions and quality of life and heighten medical and nursing effects in DPN patients.
