摘　　要：目的探讨基于团队资源管理模式建立主动脉夹层(aortic dissection,AD)绿色通道的实践与应用效果。方法采用便利抽样法选取2017年1月至2018年12月南京大学医学院附属鼓楼医院救护的483例AD患者为研究对象,其中2017年1-12月救护的213例AD患者为对照组,采用传统救治模式救护;2018年1-12月救护的270例AD患者为观察组,采用团队资源管理模式进行救护。评价并比较两组患者的CT血管造影术(computed tomography angiography,CTA)检查时间、急诊滞留时间、抢救成功率以及实施团队资源管理模式前后急诊医护人员的安全态度。结果应用团队资源管理模式后,观察组患者的CTA检查时间、急诊滞留时间明显短于对照组(均P<0.05);抢救成功率的差异无统计学意义(P>0.05);急诊医护人员安全态度6个维度的得分均有所提升,差异均有统计学意义(均P<0.05)。结论基于团队资源管理模式建立AD绿色通道的实践,有利于缩短AD患者的CTA检查时间及急诊滞留时间,提高急救品质,保障患者安全。Objective To explore the effects of the green channel of aortic dissection(AD) based on team resource management mode.Methods By convenience sampling method, 483 patients with AD who were rescued in the hospital from January 2017 to December 2018 were selected.Among them,213 AD patients rescued by traditional treatment mode from January to December 2017 were selected into the control group,and 270 AD patients rescued by team resource management mode from January to December 2018 were selected into the observation group. The time of computed tomography angiography(CTA),the time of emergency stay,the success rate of rescue and the safety attitude of emergency medical staff before and after the implementation of team resource management mode were evaluated and compared.Results After applying the team resource management mode to establish a green channel of aortic dissection,the time of CTA examination and emergency stay of the observation group were significantly lower than those of the control group( all P<0.05);there was no significant difference in the success rate of rescue(P>0.05);the scores of the six dimensions of safety attitude of emergency medical staff were statistically significant(all P<0.05).Conclusions The establishment of aortic dissection green channel based on team resource management mode reduces the time of CTA examination and emergency stay, helps the patients to get the best specialist or surgical treatment on time,improves the quality of emergency treatment,promotes the medical staff’s attitude to safety culture,and even ensures the safety of patients.
