摘  要:目的探讨256层螺旋CT诊断穿透性动脉粥样硬化性溃疡(PAU)的价值。方法回顾分析42例256层螺旋CT诊断PAU的影像资料,观察薄层轴位图像,然后根据病变进行MPR、CPR、MIP、VR重建,明确PAU的部位、形态、数目并测量大小,观察合并症情况。结果42例共显示主动脉溃疡92个,乳头状20个,蘑菇状28个,指状18个,半圆形14个,不规则形12个,溃疡大小:口径2~18mm,深度2~21mm。单发溃疡18例,多发溃疡24例,合并髂动脉病变20例,42例合并壁内血肿24例;合并局限夹层4例,并发假性动脉瘤2例。结论256层螺旋CT高速、大范围、高分辨率的优势在PAU的诊断中发挥重要的价值,可获得完整、准确的影像信息,为临床治疗方案的确定提供更大的帮助。Objective To evaluate the performance of 256-slice Spiral CT Angiography in diagnosing of penetrating atherosclerotic ulcers(PAU).Methods The imaging data of 42 patients with PAU diagnosed by 256-slice Spiral CT Angiography were retrospectively analyzed.More attention was paid to the axial thin layer image and then based on the lesions,reconstruction of MPR,CPR,MIP,VR were completed.The posittion,shape,numbers,size and secondary changes of PAU were assessed by two imaging physicians.Results 92 PAU with a diameter range of 2~18 mm in neck and 2~21 mm in depth were revealed on 256-slice Spiral CT Angiography,which presented as a focal contrast filled out pouching of the aortic wall,including 20 papillary ulcers,28 mushroom ulcers,18 finger ulcers,14 semicircular ulcers and 12 irregular ulcers.Single ulcer was found in 18 patients,and multiple lesions in 24 patients.Iliac artery involved in 20 patients,intramural hematoma was found in 24 patients,local dissecting aneurysm in 4 patients and pseudoaneurysm in 2 patients.Conclusion 256-slice Spiral CT Angiography performed well in illustration PAU with high speed,large scan area and high resolution,and facilitated the diagnosis of PAU with comprehensive imaging information.
