急性脑梗死患者心率变异性的变化特点

摘　　要：目的分析急性脑梗死患者的心率变异性(hate rate variability,HRV)变化。方法选取2018年11-12月经神经内科确诊急性脑梗死的58例患者,其中男性40例,女性18例,另选44例为对照组以对比两组HRV,并根据不同年龄段,分析急性脑梗死患者的HRV变化特点。结果急性脑梗死患者的NN间期标准差(standard deviation NN,SDNN)、NN间期平均值的标准(standard deviation NN average,SDANN)、相邻NN间期差值均方的平方(square mean squared difference adjacentNNintervals,RMSSD)及NN间期标准差指数(standard deviationNN-indx,SDNN-indx)分别为(89.83±33.29)ms、(77.86±30.63)ms、(22.38±13.51)ms、(39.43±19.15)ms,低于对照组(127.32±36.27)ms、(117.91±34.33)ms、(27.91±13.75)ms、(47.47±12.50)ms,差异具有统计学意义(t值分别为18.986、15.425、6.038、5.012,P均<0.05);中年组SDNN、SDANN分别为(86.80±26.80)ms(、72.30±24.02)ms,老年组分别为(77.83±24.94)ms(、67.00±21.97)ms,均低于青年组(105.58±29.53)ms、(92.67±28.02)ms,差异具有统计学意义(P均<0.05)。结论急性脑梗死可使患者的自主神经功能受损,影响心率变异性,且随年龄增大,对心率变异性影响更明显。Objective To analyze the changes of heart rate variability(HRV) in patients with acute cerebral infarction. Methods A total of 58 patients with acute cerebral infarction confirmed by neurology from November 2018 to December 2018 were enrolled, including 40 males and 18 females. And another 44 control subjects were enrolled. To study the Change characteristics of HRV of patients with acute cerebral infarction,two groups were analyzed by comparing HRV, and HRV of different age groups in acute cerebral infarction were also analzed. Results The standard deviation NN(SDNN), the standard deviation NN average(SDANN), and the square mean squared difference of the adjacent NN interval in patients with acute cerebral infarction The adjacent NN intervals(RMSSD) and the NN interval standard deviation index(standard deviation NN-indx, SDNN-indx) are(89.83±33.29)ms,(77.86±30.63)ms,(22.38±13.51)ms,( 39.43±19.15)ms, respectively. Significantly lower than the control group(127.32±36.27)ms,(117.91±34.33)ms,(27.91± 13.75)ms,(47.47± 12.50)ms, the difference was statistically significant(t values were 18.986, 15.425, 6.038, 5.012, P<0.05);MESN and SDANN in the middleaged group were(86.80±26.80)ms and(72.30±24.02)ms, respectively(77.83±24.94)ms and(67.00±21.97)ms, which were significantly lower than the young group(105.58±29.53)ms,(92.67±28.02)ms, the difference was statistically significant(P<0.05). Conclusion Acute cerebral infarction can impair the autonomic function of the patient and affect the heart rate variability. The older the age, the greater the impact on heart rate variability.
