不同麻醉方式在下肢骨科手术患者术中的应用

摘　　要：目的探讨下肢骨科手术中利用不用麻醉方式得到的不同镇痛效果,以选择合理麻醉方式进行手术。方法选择2018年4月-2019年2月间曾在我院接受下肢骨科手术的患者106例,分为观察组与对照组表示这些患者,对照组患者在手术中选择全身麻醉方式,观察组患者选择硬膜外麻醉方式,观察两组患者麻醉效果。结果在手术结束之后,观察两组患者不同时间的VAS疼痛评分,结果显示观察组患者评分均低于对照组;观察两组患者的尿管留置率,观察组患者的尿管留置率为5.7%,对照组中患者的尿管留置率为22.6%;观察两组患者的镇痛效果,结果显示观察组中患者的镇痛评分明显优于对照组,两组患者之间存在明显差异。结论在下肢骨科手术中全身麻醉与硬膜外麻醉属于应用较广泛的麻醉方式,其中硬膜外麻醉方式的镇痛效果比较理想,并且在手术后可使患者尽快自行排尿。Objective To investigate the different analgesic effects obtained without anaesthesia in orthopaedic surgery of lower extremity, and to choose the rational anaesthesia method. Methods 106 patients who had received orthopaedic surgery in the lower extremities from April 2018 to February 2019 were selected. These patients were represented by the observation group and the control group respectively. The control group chose general anesthesia during the operation. The patient chose epidural anesthesia in the observation group. The anesthetic effect of two groups of patients was observed. Results After the operation, VAS pain score was observed at different time in two groups. The results showed that the patients in the observation group scored lower than the control group. The retention rate of urinary tract was 5.7% in the observation group and 22.6 % in the control group. The results showed that the analgesic score of the observe groups was better than that of the control group. There were significant differences between the two groups. Conclusion General anesthesia and epidural anesthesia are widely used in orthopaedic surgery of the lower extremities. The analgesic effect of epidural anesthesia is ideal, and the patient can urinate as soon as possible after surgery.
