苯磺酸氨氯地平联合厄贝沙坦氢氯噻嗪对左室肥厚老年高血压患者左室舒张功能的影响研究

摘　　要：目的:研究苯磺酸氨氯地平联合厄贝沙坦氢氯噻嗪对左室肥厚老年高血压患者左室舒张功能的影响。方法:以左室肥厚老年高血压患者72例为研究对象,按照数字表随机分组法分为A组、B组与C组各24例,A组采用苯磺酸氨氯地平疗法,B组采用厄贝沙坦氢氯噻嗪疗法,C组采用苯磺酸氨氯地平联合厄贝沙坦氢氯噻嗪疗法。对比三组患者的收缩压、舒张压及左室舒张功能水平差异。结果:干预前三组患者的舒张压与收缩压水平均差异无统计学意义(P>0.05),治疗后三组的舒张压与收缩压水平均下降,前后相比,差异有统计学意义(P<0.001),但C组舒张压与收缩压水平低于A组与B组,差异有统计学意义(P<0.001)。治疗前三组患者各项指标水平差异无统计学意义(P>0.05),治疗后各组指标水平均有所改善,差异有统计学意义(P<0.05),但A组与B组治疗后指标水平差异无统计学意义(P>0.05),A组、B组指标水平均差于C组,差异有统计学意义(P<0.05)。A组与B组总有效率差异较小,差异无统计学意义(P>0.05),但C组总有效率显著高于A组与B组,数据相比差异有统计学意义(P<0.001)。结论:苯磺酸氨氯地平联合厄贝沙坦氢氯噻嗪疗法较苯磺酸氨氯地平与厄贝沙坦氢氯噻嗪单用效果更好且苯磺酸氨氯地平与厄贝沙坦氢氯噻嗪单用效果差异较小。Objective To study the effects of amlodipine besylate combined with irbesartan hydrochlorothiazide on left ventricular diastolic function in elderly hypertensive patients with left ventricular hypertrophy.Method 72 elderly hypertensive patients with left ventricular hypertrophy in our hospital were randomly divided into group A,group B and group C according to the digital table.24 cases in each group.Group A was treated with amlodipine besylate,group B with irbesartan hydrochlorothiazide and group C with amlodipine besylate.Diazoxide combined with irbesartan hydrochlorothiazide therapy.The differences of systolic blood pressure,diastolic blood pressure and left ventricular diastolic function between the three groups were compared.Results There was no significant difference in diastolic blood pressure and systolic blood pressure between the three groups before intervention(P>0.05).After treatment,the diastolic blood pressure and systolic blood pressure of the three groups decreased,compared with before treatment(P<0.001),but the diastolic blood pressure and systolic blood pressure of group C were lower than those of group A and group B,the difference was statsitically significant(P<0.001).There was no significant difference between the three groups before treatment(P>0.05).After treatment,the indexes of each group were improved(P<0.05),but there was no significant difference between group A and group B(P>0.05).The indexes of group A and group B were significantly lower than those of group C(P<0.05).The difference of total effective rate between group A and group B was small(P>0.05),but the total effective rate of group C was significantly higher than that of group A and group B(P<0.001).Conclusion Amlodipine besylate combined with irbesartan hydrochlorothiazide is more effective than amlodipine besylate and irbesartan hydrochlorothiazide alone,and there is little difference between amlodipine besylate and irbesartan hydrochlorothiazide.
