成都市某社区“中重度”失能老人对老年护理医院需求的影响因素分析

摘　　要：目的调查社区"中重度"失能老人生活现状以及他们对老年护理医院的需求愿望及其影响因素。方法采用整群抽样方法,对本单位所属社区卫生服务中心辖区内所有失能老人,符合条件者采用功能独立性评测(FIM)量表、健康调查简表(SF-36)及自制调查问卷进行面对面评估及调查,对收集的资料进行Logistic回归分析。结果共调查具有完整资料的老人420人,平均年龄(79.18±9.16)岁,男53.9%,女46.1%;中重度失能老人占失能老人的18.5%。FIM分类中度、重度、极重度和完全依赖者分别为42.6%、42.4%、11.9%和3.1%。多元回归分析显示:文化程度、失能程度(FIM分类)、目前生活质量、每天需要协助护理的项目数、2月内对医疗及护理的需求、家人每月能给与的最大经济支持、每月医疗费用、每月总费用的支出等是影响社区"中重度"失能老人选择是否入住老年护理医院的独立因素。结论成都市某社区60岁及以上老人中重度失能发生率高,老年护理医院可以为中重度失能、多病老人提供医疗、康复、照护、护理为一体的综合服务,但老人入住受多种因素影响,如何提升中重度失能老人的生活质量面临诸多挑战。Objective To investigate the living status of the elderly with moderate or severe disability in the community,their demands and aspirations for geriatric nursing hospitals and the influencing factors.Methods The cluster sampling method was used to conduct the face-to-face assessment and investigation among all the disabled elderly in the area under the jurisdiction of the community health service center of Chengdu Elderly Care Hospital.The eligible elderly were assessed face-to-face with the functional independence measure(FIM),SF-36 scale and self-made questionnaire.The Logistic regression analysis was used to analyze the collected data.Results A total of420 elderly people with valid data were surveyed with the average age of(79.18±9.16)years.53.9%of them were male and 46.1%were female.The elderly with moderate or severe disability accounted for 18.5%.The moderately,severely,extremely severely and completely dependent elderly accounted for 42.6%,42.4%,11.9%and 3.1%respectively according to the FIM classification.The results of the multiple regression analysis showed that The educational level,disability degree according to the FIM classification,current quality of life,number of nursing items needing assistance every day,demand for the medical treatment and nursing within 2 months,maximum financial support from the family members each month,monthly medical expense and monthly total expenses were the independent factors affecting the choice of the admission to geriatric nursing hospitals for the elderly with moderate or severe disability in the community.Conclusion The incidence of the moderate and severe disability is high among the elderly aged 60 years and above in a community of Chengdu.The geriatric nursing hospital can provide comprehensive services of medical treatment,rehabilitation,care and nursing for the elderly with moderate or severe disability and multiple diseases.However,there are many factors affecting the elderly’s admission to geriatric nursing hospitals,which means that there are many chall
