心肌淀粉样变性12例临床特点分析

摘　　要：收集2011年1月-2018年6月新疆医科大学第一附属医院诊治心肌淀粉样变性患者12例,临床表现为进行性、难治性心功能不全,其中以右心衰竭为主9例(75.0%),合并低血压6例(50.0%),多浆膜腔积液7例(58.3%);心电图呈肢体导联低电压9例(75.0%);超声心动图均提示不同程度的室间隔或左室室壁增厚,心肌回声明显增强不均匀11例,其中7例(58.0%)患者心肌内可见颗粒样强回声或闪光点;病理或组织检查均为阳性;治疗予以利尿、纠正心功能、抗凝、化疗等综合措施。病情好转6例,住院期间死亡3例,自动出院3例。Twelve patients with myocardial amyloidosis from the First Affiliated Hospital of Xinjiang Medical University from January 2011 to June 2018 were collected.The clinical manifestations are progressive and refractory cardiac dysfunction.Nine of them(75.0%) were predominantly right heart failure,6(50.0%) had hypotension,and 7(58.3%) had pleural effusion.Electrocardiogram showed low voltage in 9 cases(75.0%).Echocardiography showed different degrees of ventricular septum or left ventricular wall thickening,and myocardial echo was significantly enhanced unevenly in 11 cases,of which 7 patients(58.0%) had granular-like strong echoes or flash points.Pathological or tissue examinations were positive.Treatment should be integrated with diuretics,cardiac function correction,anticoagulation,and chemotherapy.
