参麦注射液联合呋塞米治疗心力衰竭的疗效及对血浆Hcy、NT-proBNP、瘦素的影响

摘　　要：目的探讨参麦注射液联合呋塞米治疗心力衰竭的疗效及对血浆同型半胱氨酸(Hcy)、N末端脑利钠肽前体(NT-proBNP)、瘦素的影响。方法选取2015年6月—2017年6月廊坊市中医院收治的98例心力衰竭病人,按随机数表法分为对照组和研究组,每组49例。对照组予以呋塞米治疗,研究组在对照组基础上加以参麦注射液治疗。比较两组临床疗效,观察两组治疗前后血浆Hcy、NT-proBNP、瘦素、心率、舒张压、收缩压、左室射血分数(LVEF)、左心室舒张末期容积(LVEDV)及左心室收缩末期容积(LVESV)水平变化及不良反应发生情况。结果研究组总有效率高于对照组,差异有统计学意义(89.80%与69.39%,P<0.05)。治疗后,两组血浆Hcy、NT-proBNP、瘦素水平均较治疗前下降,且研究组低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组LVEF均较治疗前上升,LVEDV及LVESV均较治疗前下降,且研究组变化更明显(P<0.05)。治疗后,两组心率、血压均较治疗前下降(P<0.05),且研究组下降较对照组更明显,差异均有统计学意义(P<0.05)。研究组、对照组不良反应比发生率比较差异无统计学意义(20.40%与10.20%,P>0.05)。结论参麦注射液联合呋塞米治疗心力衰竭,能够降低血浆Hcy、NT-proBNP及瘦素水平,改善心功能,提高临床疗效。Objective To research the efficacy of Shenmai injection combined with furosemide in the treatment of heart failure and its effects on plasma homocysteine(Hcy),N terminal brain natriuretic peptide precursor(NT-proBNP)and leptin.Methods Ninety-eight patients with heart failure in Langfang Traditional Chinese Medicine Hospital were divided into control group and research group according to random number table method,with 49 cases in each group.The patients in control group were treated with furosemide,the patients in research group was treated with Shenmai injection on the basis of the control group.The clinical curative effect,plasma Hcy,NT-proBNP,leptin,and heart rate(HR),diastolic blood pressure(DBP),systolic blood pressure(SBP),left ventricular ejection fraction(LEVF),left ventricular end-diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)levels,and adverse reactions between two groups were compared.Results The total effective rate in research group was higher than that of the control group,and the difference was statistically significant(89.80% vs 69.39%,P<0.05).After treatment,plasma Hcy,NT-proBnp,leptin levels in both groups were decreased,and the research group was lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,LVEF in both groups was increased,LVEDV and LVESV were decreased,the research group changed more significantly(P<0.05).After treatment,HR,SBP and DBP in two groups were decreased,and the research group decreased significantly,and the difference were statistically significant(P<0.05).There was no difference in adverse reactions among the two group(20.40% vs 10.20%,P>0.05).Conclusion Shenmai injection combined with furoxamide in the treatment of heart failure can reduce plasma Hcy,NT-proBNP,and leptin levels,improve cardiac function,and improve curative effect.
