中药辅助保守治疗老年阑尾脓肿的临床效果评价

摘　　要：目的:探讨中药辅助抗生素治疗老年阑尾脓肿的效果。方法:选取76例老年阑尾脓肿患者,按随机数字分组为对照组、观察组各38例。对照组单纯予抗生素治疗;观察组在抗生素治疗同时,予大黄牡丹汤合红藤煎剂加减口服及七味内消散局部外敷。统计两组患者的住院时间、发热消退时间、腹痛缓解时间、治愈率、脓肿消失时间、住院费用、腹泻发生率等指标。结果:与对照组相比,观察组平均住院时间缩短[(12.0±4.5)d vs(18.5±8.4)d],退热时间[(5.3±2.6)d vs(9.7±4.4)d]及腹痛缓解时间[(7.7±3.2)d vs(12.1±6.6)d]提前,治愈率高(86.8%vs 26.3%),脓肿消退时间缩短[(11.2±4.1)d vs(17.8±7.5)d],住院费用降低[(1.72±0.34)万元vs(2.63±0.72)万元],结果均有统计学意义。观察组出现轻度腹泻比例增高,但无统计学意义。结论:中药辅助治疗能提高抗生素治疗阑尾脓肿的效果,加速老年阑尾脓肿的康复、节约医疗资源、减轻患者经济负担,治愈率高,且无明显副作用,效果良好,安全可行。Objective To explore the effect of traditional Chinese medicine(TCM)in conservative treatment of appendiceal abscess in elderly.Methods Patients with appendiceal abscess in our department were operated with conservative treatment and divided into control group and observation group.The patients in control group were treated with antibiotics simply.Meanwhile,the patients in observation group were treated not only with antibiotics but also with TCM oral modified decoction(Rhubarb soup peony and Red vine decoction)and Seven flavor dissipation for external application.The hospitalization time,fever clearance time,abdominal pain relief time,abscess disappearing time and hospitalization expenses of the two groups were compared.Results Compared with the control group,the average hospitalization time in the observation group was shortened[(12.0±4.5)d vs(18.5±8.4)d],and the fever clearance time[(5.3±2.6)d vs(9.7±4.4)d]and abdominal pain relief time[(7.7±3.2)d vs(12.1±6.6)d]were decreased.The abscess disappeared faster[(11.2±4.1)d vs(17.8±7.5)d]and the hospitalization expenses were lower[(1.72±0.34)×104 Yuan vs(2.63±0.72)×104 Yuan],with statistically significant differences(P<0.01).But the patients in observation group had increased incidence of mild diarrhea.Conclusions TCM treatment can accelerate the rehabilitation of appendiceal abscess in elderly,save medical resources,reduce the economic burden of patients,and has a high cure rate without obvious side effects.It has good effect and is safe and feasible.
