摘  要:目的了解2015—2018年北京市西城区HIV抗体筛查有反应标本的确证结果,为艾滋病的发现、防治提供数据支持。方法2015—2018年,对北京市西城区HIV抗体筛查有反应标本使用MP生物亚太公司人类免疫缺陷病毒(HIV 1+2型)抗体检测试剂盒(免疫印迹法,WB)进行确证检测。结果对3494份HIV抗体筛查有反应标本进行WB确证试验发现,HIV-1抗体阳性标本1822份(52.15%);HIV抗体不确定标本416份(11.91%);HIV抗体阴性标本1256份(39.95%)。HIV-1抗体阳性标本中,条带gp120、gp160、p24、gp41出现频率较高,均>99.00%,条带p55出现1003份,频率最低,为55.05%。HIV抗体不确定标本中,条带p24共357份,出现频率最高,为85.82%。男女HIV-1抗体阳性感染率比较,差异有统计学意义(P<0.05)。不同性别间HIV抗体不确定标本中,带型gp160、gp120反应率比较,差异均有统计学意义(均P<0.05)。不同年龄间HIV-1抗体阳性条带中,带型p51、p24反应率比较,差异均有统计学意义(均P<0.05)。不同年龄HIV抗体不确定标本中,带型gp160、gp120、gp41、p24反应率比较,差异均有统计学意义(均P<0.05)。结论WB法作为HIV抗体检测的“金标准”,其稳定性、可靠性(特异性、准确性)已得到广泛认可,此方法能够准确判断艾滋病病毒感染。但人类免疫系统复杂多样,导致HIV抗体不确定标本中各条带出现频率差异存在不确定性,需要在后续的研究中对HIV抗体不确定标本提高警惕,结合流行病学调查、核酸检测及定期随访复检等方式进一步追踪以确证结果。Objective To know the confirmatory results of HIV antibody detected by Western blot in Xicheng District of Beijing from 2015-2018,provide data support for AIDS detection,prevention and treatment.Methods From 2015-2018,all the positive HIV antibody primary screening samples were detected with Western blot(WB)by using the human immunodeficiency virus(HIV 1+2)antibody detection kit produced by MP Biomedicals Asia Pacific Company.Results In 3494 cases of WB test,there were 1822 samples confirmed to be HIV-1(+),accounting for 52.15%,416 samples were uncertain,accounting for 11.91%,1256 samples were negative,accounting for 39.95%.In HIV-1 antibody positive samples,the frequency of gp120,gp160,p24 and gp41 were higher than 99.00%,while the frequency of p55 was lowest,which was in 1003 cases,accounting for 55.05%.In HIV-1 antibody uncertain samples,the frequency of p24 was highest,which was in 357 cases,accounting for 85.82%.The difference in positive rate of HIV-1 antibody between male and female was statistically significant(P<0.05).The uncertain results of WB confirmations indicated that the response rates of gp160 and gp120 were statistically significant between different genders(all P<0.05).In HIV-1 antibody positive samples,there were statistically significant differences in the response rates of p51 and p24 among different age groups(all P<0.05).In HIV-1 antibody uncertain samples,there were statistically significant differences in the response rates of gp160,gp120,gp41 and p 24 among different a ge groups(all P<0.05).Conclusion As the"gold standard"of HIV antibody detection,the stability and reliability of WB method have been widely recognized,which can accurately determine HIV infection.However,due to the complexity and diversity of human immune system,there is uncertainty in the frequency difference of each band in the HIV antibody uncertainty cases,which requires us to be more alert to the antibody uncertainty samples in the follow-up studies,and to track the progress by combining epidemiological investigation
