摘　　要：目的 观察过敏性紫癜性肾炎(HSPN)的临床病理特征,检测组织中微小RNA-34b(mi R-34b)的表达,分析其与白细胞介素-6(IL-6)的关系。方法 前瞻性选择49例HSPN患者作为观察组,均行肾穿刺病理活检,选择10例微小病变性肾小球病的穿刺组织作为对照组。观察HSPN的光镜、免疫荧光及电镜特征。应用实时荧光定量PCR法检测两组中mi R-34b的表达,观察与临床病理特征的关系。应用Western Blot法检测观察组中IL-6的表达,分析mi R-34b与IL-6的相关性。结果 49例患儿均具有典型的形态特征,其中I型1例,II型10例,IIIa型17,IIIb型19例,IV型1例,V型1例。观察组mi R-34b的表达明显低于对照组(P <0. 05);观察组有新月体形成组mi R-34b的表达量明显低于无新月体形成组(P <0. 05);随着病理分型的增高mi R-34b的表达呈降低趋势(P <0. 05)。相关分析显示mi R-34b与IL-6具有负相关性(P <0. 05)。结论 HSPN病理特征是以Ig A沉积为主,组织中mi R-34b的表达降低,且与病变严重程度有关。mi R-34b的作用可能与对IL-6的调节有关。Objective To observe the clinicopathological features of Henoch-Schonlein Purpura Nephritis(HSPN),pay attention to the expression of microRNA-34b(miR-34b)in tissues,and analyze its relationship with interleukin-6(IL-6).Methods 49 patients with HSPN were selected as the observation group.All patients underwent renal biopsy,and 10 patients with minimally invasive glomerulopathy were selected as the control group.The light,immunofluorescence and electron microscopic characteristics of HSPN were observed.MiR-34b were detected by real-time fluorescence quantitative PCR in the two groups.IL-6 was detected by Western Blot method in the observation group.The correlation between miR-34b and IL-6 was analyzed.Results 49 cases had typical morphological characteristics,including 1 case of type I,10 cases of type II,17 cases of type IIIa,19 cases of type IIIb,1 case of type IV and 1 case of type V.The expression of miR-34b was significantly lower in the observation group than that in the control group.The expression of miR-34b was significantly different in different pathological types and crescent formation in the observation group.Negative correlation were found between miR-34b and IL-6 by correlation analysis.Conclusion The pathological features of HSPN are mainly IgA deposition,and the expression of miR-34b is decreased,which is related to the severity of HSPN.The effect of miR-34b may be related to the regulation of IL-6.
