CD4^+T淋巴细胞绝对计数检测在孕妇胎儿畸形筛查中的应用

摘　　要：目的:探讨CD4^+T淋巴细胞绝对计数检测在孕妇胎儿畸形筛查中的应用。方法:选取2016年1月-2018年12月于笔者所在医院超声科进行胎儿畸形筛查的孕妇4850例,行三维超声检查,对筛查后发现胎儿畸形的孕妇在征求其同意后进行CD4^+T淋巴细胞绝对计数测定,并对同时期超声筛查胎儿正常的孕妇同时进行CD4^+T淋巴细胞绝对计数测定。结果:4850例孕妇中有56例经超声筛查发现胎儿畸形,畸形率为1.15%,胎儿畸形类型以中枢神经系统畸形、泌尿系统及心血管系统畸形为主,其中中枢神经系统畸形以脑积水及无脑为主,泌尿系统畸形以肾积水为主;心血管系统畸形以室间隔缺损为主。胎儿畸形孕妇组CD4^+T淋巴细胞绝对计数低于胎儿正常孕妇组,差异有统计学意义(P<0.05)。结论:超声检查胎儿畸形的孕妇CD4^+T淋巴细胞绝对计数较胎儿正常的孕妇低,CD4^+T淋巴细胞绝对计数检测可能有助于孕妇胎儿畸形的筛查。Objective:To explore the application value of CD4^+T lymphocyte absolute count in fetal malformation screening of pregnant women.Method:A total of 4850 pregnant women who were screened for fetal malformation in the ultrasound department of our hospital from January 2016 to December 2018 were selected.Three-dimensional ultrasound examination was performed.The pregnant women who found fetal malformation after screening were measured for CD4^+T lymphocyte absolute count after asking for their consent,and the pregnant women who were screened for fetal normal by ultrasound at the same time were measured for CD4^+T lymphocyte absolute count.Result:Among the 4850 pregnant women,56 cases were found to have fetal malformations by ultrasound screening.The malformation rate was 1.15%.The fetal malformations were mainly central nervous system malformations,urinary system and cardiovascular system malformations.The central nervous system malformations were were mainly hydrocephalus and anencephaly,and urinary system malformations were mainly hydrocephalus.The main malformations of cardiovascular system were ventricular septal defect.The absolute count of CD4^+T lymphocytes in pregnant women with fetal malformation was lower than that in pregnant women with normal fetuses,and the difference was statistically significant(P<0.05).Conclusion:The absolute count of CD4^+T lymphocytes in pregnant women with fetal malformation screened by ultrasound was lower than that in pregnant women with normal fetuses,and the absolute count of CD4^+T lymphocytes might be helpful for the screening of fetal malformation.
