摘 要:目的:探讨妊娠中、晚期孕妇TSH水平与新生儿TSH水平的关系。方法:选择2016年8月-2018年8月厦门市同安区妇幼保健院妊娠中、晚期孕妇80例,将非碘缺乏地区孕妇及碘营养不足地区孕妇分别纳入研究A组和研究B组;选择同时间段内笔者所在医院正常非孕妇30例,纳入对照组。对比三组TSH、UI水平及研究A、B组新生儿TSH水平。结果:研究A组妊娠中期TSH水平低于研究B组,差异有统计学意义(P<0.05)。两组妊娠晚期TSH水平对比,差异无统计学意义(P>0.05)。研究A组妊娠中、晚期TSH水平均高于对照组,差异均有统计学意义(P<0.05)。临产后,研究A组UI水平高于研究B组,差异有统计学意义(P<0.05)。研究B组新生儿TSH水平高于研究A组,差异有统计学意义(P<0.05)。结论:检测妊娠中晚期孕妇的TSH水平能够全面评估胎儿的碘营养情况,指导孕妇在孕期合理化补充碘物质,能够有效预防新生儿甲状腺功能低下。Objective:To investigate the relationship of TSH levels of pregnant women in the second and third trimester of pregnancy and neonatals.Method:A total of 80 pregnant women in the second and third trimester of pregnancy in Xiamen Tong’an District Maternal and Child Health Hospital from August 2016 to August 2018 were selected,and pregnant women in non-iodine-deficient areas and pregnant women in iodine-deficient areas were included in the study group A and the study group B,respectively.And 30 cases of normal non-pregnant women in the same period of time were selected and included in the control group.The levels of TSH and UI in three groups were compared,and the levels of TSH of neonatals in the study group A and the study group B were studied.Result:In the second trimester of pregnancy,TSH level of the study group A was lower than that of the study group B,and the difference was statistically significant(P<0.05).TSH level was compared between the two groups in the third trimester of pregnancy,and the difference was not statistically significant(P>0.05).In the second and third trimester of pregnancy,TSH levels of the study group A were higher than those of the control group,and the differences were statistically significant(P<0.05).After labor,the UI level of the study group A was higher than that of the study group B,and the difference was statistically significant(P<0.05).TSH level of neonatals in the study group B was higher than that of the study group A,and the difference was statistically significant(P<0.05).Conclusion:The detection of TSH level of pregnant women in the second and third trimester of pregnancy can comprehensively evaluate the iodic nutritional status of the fetus,and guide pregnant women to rationalize iodine supplement during pregnancy,which can effectively prevent neonatal hypothyroidism.
