摘　　要：目的:探讨在剖宫产同时行子宫肌瘤剥除术中,先预防性结扎双侧子宫动脉上行支后再行子宫肌瘤剥除的效果。方法:将2015年6月-2018年6月妊娠合并子宫肌瘤并选择剖宫产终止妊娠孕妇60例作为研究对象,随机分为研究组及对照组,各30例。研究组胎盘娩出后,先预防性结扎双侧子宫动脉上行支后再行子宫肌瘤剥除术;对照组剖宫产后直接行子宫肌瘤剥除术。观察两组术中出血量、分娩后24 h产后出血率及手术时间。结果:研究组术中出血量明显少于对照组,产后出血率低于对照组,手术时间短于对照组,差异均有统计学意义(P<0.05)。结论:在剖宫产同时行子宫肌瘤剥除术中,先预防性结扎双侧子宫动脉上行支后再剥除子宫肌瘤能够减少术中出血量,降低产后出血率,节省手术时间,值得临床推广应用。Objective:To explore the effect of uterine leiomyomas were removed after prophylactic ligation of the ascending branch of uterine artery in myomectomy during cesarean section.Method:A total of 60 cases of pregnancy complicated with uterine leiomyomas and terminating pregnancy through cesarean section were selected as the study objects from June 2015 to June 2018.Pregnant women were randomly divided into the study group and the control group,30 cases in each group.In the study group,after placenta delivery,myomectomy was implemented after prophylactic ligation of the ascending branch of uterine artery.While in the control group,myomectomy was implemented after cesarean section.The amount of intraoperative bleeding,rate of postpartum bleeding within 24 hours after delivery and operation time were compared between two groups.Result:The amount of intraoperative bleeding in the study group was significantly less than that of the control group,and the rate of postpartum bleeding was lower than that of the control group,and the operation time was shorter than that of the control group,and the differences were statistically significant(P<0.05).Conclusion:Uterine leiomyomas were removed after prophylactic ligation of the ascending branch of uterine artery in myomectomy during cesarean section can reduce the amount of intraoperative bleeding,reduce the rate of postpartum bleeding and save operation time,which is worthy of clinical application.
