CT与B超对急性阑尾炎诊断的临床应用价值对比研究

摘　　要：目的:对比CT与B超在临床诊断和鉴别诊断急性阑尾炎的应用价值。方法:随机选择2017年1月至2018年5月期间以急性阑尾炎作为诊断进行治疗及手术的患者68例,均进行CT及B超检查,以及术后病理作为最终诊断结果。对比观察CT与B超对急性阑尾炎诊断的符合率、灵敏度和特异度。结果:68例患者最终病理确诊60例,CT的符合率、灵敏度和特异度(分别为94.12%、93.33%、100%)均高于B超检测的符合率、灵敏度和特异度(分别为73.53%、76.67%、50%),差异具有统计学意义(P<0.05)。结论:B超检查方便快捷、费用较低、且患者没有暴露射线的顾虑,但与CT比较具有相对较低的符合率、灵敏度、特异度,因此,本次研究建议针对B超检查为阴性的疑似病例可以进一步开展CT检查,以降低误诊、漏诊率,同时又能够进一步查找病因。Objective:To compare the clinical value of CT and B-ultrasound in diagnosis and differential of appendicitis.Methods:A total of 68 patients with acute appendicitis diagnosed and treated between 2017 and 2018 were randomly selected. CT and B-ultrasound examinations were performed, and postoperative pathology was used as the final diagnosis. To compare the accuracy, sensitivity and specificity of CT and B-ultrasound in the diagnosis of acute appendicitis.Results:60 cases were diagnosed with final pathology. The Accuracy, sensitivity and specificity of CT( respectively: 94.12%, 93.33%, 100%) were higher than those of B-ultrasound(respectively: 73.53%, 76.67%, 50%). The difference was statistically significant(P<0.05).Conclusion:B-ultrasound has the advantages of convenience, low cost, and no exposure to radiation. However, compared with CT, b-ultrasound examination has relatively low coincidence rate, sensitivity and specificity. Therefore, this study suggests that further CT examination can be carried out for suspected cases with negative b-ultrasound examination, so as to reduce the rate of misdiagnosis and missed diagnosis and further find out the cause.
