摘 要:目的探讨无局麻超声引导下改良塞丁格技术在PICC置管术中的应用效果.方法选择有PICC置管适应证且无置管禁忌证的住院患者300例,将2017年1月至2017年6月使用局部麻醉超声引导下改良塞丁格技术PICC置管的150例患者设为对照组,将2018年1月至2018年6月PICC进行无局麻超声引导下改良塞丁格技术PICC置管的150例患者设为实验组,分析两组患者疼痛评分、置管成功率、穿刺成功率等置管效果,探究其临床操作可行性.观察比较两组患者1次置管成功率、置管舒适度评分、置管后并发症发生率.结果实验组较对照组1次置管成功率高、患者置管舒适度评分较高,差异均具有统计学意义(P<0.05);实验组与对照组置管后并发症发生率比较差异无统计学意义.结论采用无局麻超声引导下改良塞丁格技术进行PICC置管1次置管成功率高,同时能提升患者置管舒适度评分,且并不增加患者并发症发生率.Objective To investigate application of modified Sedinger technique in PICC catheterization without local anesthesia. Methods A total of 300 patients with PICC catheterization indications and no contraindications for catheterization were enrolled, 150 patients who underwent lo- cal anesthesia-guided ultrasound-guided modified Singer technique PICC catheterization from January 2017 to June 2017 as the control group, 150 patients who underwent PIMC catheterization without modified local anesthesia under the guidance of local anesthesia with ultrasound from January 2018 to June 2018 were enrolled as the experimental group. The pain score and catheter success rate of the two groups were analyzed, puncture suc- cess rate and other tube placement effects, to explore its clinical operational feasibility. The success rate of one-time catheterization was compared be- tween the two groups, the catheter comfort scores of the two groups were compared, and the complication rate after catheterization was observed. Results Compared with the control group, the experimental group had a higher success rate of catheterization and higher comfort scores. The differ- ence was statistically significant (P<0.05). The difference in the incidence of complications between the experimental group and the control group was compared and not statistically significant. Conclusion The modified Sedinger technique without local anesthesia is used to perform a high suc- cess rate of PICC catheterization, and to improve the patient' s catheter comfort score without increasing the incidence of complications.
