摘　　要：有效整合养老和医疗资源,建立持续照护型养老设施,提供老年人从自理—介助—介护—失能不同健康状态的全过程可持续长期照护;养照中心建立完善的诊疗、护理、膳食、学习、及休闲娱乐活动等功能,及以老年人的自理能力差异和对医护的不同要求分别配备相应设施的养护单元,建立起对住宿型老人的服务体系,又成为周边社区养老体系的有效支撑。建立以居家为基础、社区为依托、照护型养老设施为支撑、医养护相结合,结构合理、功能完善、服务优良的新型养老服务体系。Cangnan Nursing centers effectively integrate old-age care and medical resources,establish sustainable care-type old-age care facilities,and provide sustainable long-term care for the elderly from the whole process of self-help-mediation-care-disability to different health states.The Nursing Center has established a sound diagnosis and treatment,nursing,diet,learning,leisure and recreational activities,and the self-care ability of the elderly and the requirements for medical care are equipped with corresponding maintenance units,and the service system for the elderly living in the residential area is established,which has become an effective support for the old-age care system in the surrounding communities.A new pension service system with reasonable structure,perfect function and excellent service should be established,which is based on home,community,care-oriented pension facilities and combined with medical care.
