“云随访”与传统随访模式在尿路造口患者出院康复中的应用比较

摘　　要：目的:设计并实施针对尿路造口患者的“云随访”方案,与传统随访模式进行比较。方法:将2016年10月1日~2018年10月31日行全膀胱切除+尿流改道术的80例患者随机分为观察组和对照组各40例。按需求为观察组设计“云随访”方案并实施,对照组进行传统随访。比较两组出院当时、出院后1个月及3个月的造口患者生命质量、造口自我护理能力、造口自我效能及随访满意度变化情况。结果:出院当时,两组生命质量总分及心理、社会维度得分比较差异有统计学意义(P<0.05);出院后1个月,两组生命质量精神维度,造口自我护理能力,造口自我效能得分及随访满意度比较差异有统计学意义(P<0.05);出院后3个月,两组生命质量总分及社会、精神维度,造口自我护理能力,造口自我效能得分及随访满意度比较差异有统计学意义(P<0.05)。结论:针对尿路造口患者实施“云随访”方案,对改善患者生命质量,提高造口自我护理能力、自我效能及随访满意度均具有积极意义,相对于传统随访模式,更能有效反映患者身心感受,值得临床推广使用。Objective:To design and implement a cloud follow-up program for patients with urostomy,and compare it with the traditional follow-up model.Methods:From October 1,2016 to October 31,2018,80 patients who underwent total cystectomy+urinary diversion were randomly divided into the observation group and the control group with 40 cases each.The cloud follow-up scheme was designed and implemented for the observation group according to the requirements,while the control group was followed up by traditional follow-up.The changes of life quality,self-nursing ability,self-efficacy and follow-up satisfaction of patients with stoma at discharge,1 month and 3 months after discharge were compared between the two groups.Results:At the time of discharge,there were statistically significant differences in the total score of quality of life and the scores of psychological and social dimensions between the two groups(P<0.05).There were statistically significant differences in the spiritual dimension of life quality,self-care ability of stoma,score of stoma self-efficacy and follow-up satisfaction between the two groups 1 month after discharge(P<0.05).Three months after discharge,there were statistically significant differences in the total score of life quality,social and spiritual dimensions,stoma self-care ability,stoma self-efficacy score and follow-up satisfaction between the two groups(P<0.05).Conclusion:The implementation of cloud follow-up program for patients with urostomy is of positive significance in improving the quality of life of patients,self-care ability,self-efficacy and follow-up satisfaction of patients.Compared with the traditional follow-up model,it is more effective in reflecting patients′physical and mental feelings,and worthy of clinical promotion.
