摘  要:目的:探讨基质金属蛋白酶-3(MMP-3)、MMP-7、MMP-12基因突变与老年慢性阻塞性肺疾病(COPD)发生风险的相关性。方法:选取2017年11月至2019年4月,在本院就诊的108例COPD患者为COPD组,另选取同期门诊体检的80例健康体检者为对照组。采用聚合酶链式反应-限制性片段多态性(PCR-RFLP)法检测MMP-3基因rs679620位点、MMP-7基因rs11568818位点及MMP-12基因-1082位点的基因分型,比较不同基因型与老年COPD发生风险的关系。Hardy-Weinberg遗传平衡定律验证两组基因型频率是否具有代表性。收集老年COPD患者肺功能及疾病严重程度相关参数并分析其与各基因型的关系。采用Logistic回归法分析影响COPD发生风险的相关因素。结果:MMP-3基因rs679620位点G→A基因突变,MMP-7基因rs11568818位点A→G基因突变,MMP-12基因-1082位点A→G基因突变,各基因型分布均符合Hardy-Weinberg定律。COPD组与对照组MMP-3基因G、A等位基因突变频率分布比较差异无统计学意义(P>0.0167),MMP-7基因G、A等位基因突变频率显著高于对照组(P<0.0167),MMP-12基因G、A等位基因突变频率显著高于对照组(P<0.0167);携带MMP-3基因A突变基因、MMP-7基因G突变基因、MMP-12基因G突变基因的COPD患者吸烟指数、支气管壁厚度、支气管壁分级均分别显著高于GG基因型、AA基因型患者(P<0.05),而相邻动脉直径显著降低(P<0.05);Logistic分析显示MMP-3 AA、MMP-7 GG及MMP-12 GG突变基因型均为COPD发生的危险因素。结论:MMP-3、MMP-7、MMP-12基因突变可能与老年COPD发生有关,突变基因型AA(MMP-3)、GG(MMP-7)、GG(MMP-12)均可增加老年COPD发生风险。Objective:To explore the correlations between the mutations of matrix metalloproteinase-3(MMP-3),matrix metalloproteinase-7(MMP-7),matrix metalloproteinase-12(MMP-12)genes with the risk of chronic obstructive pulmonary disease(COPD)in the elderly.Methods:108 COPD patients treated in our hospital from November 2017 to April 2019 were selected as COPD group,and 80 Non-COPD were selected as control group.Polymerase chain reaction-restriction fragment polymorphism(PCR-RFLP)was used to detect the genotypes of the rs679620 locus of MMP-3 gene,the rs11568818 locus of MMP-7 gene,the-1082 locus of MMP-12 gene,and compared the relationships between different genotypes with the risk of COPD in the elderly.Hardy-Weinberg’s law of genetic balance verified whether the genotype frequencies of the two groups are representative or not.Pulmonary function and disease severity related indexes in elderly patients with COPD were collected and their relationships with various genotypes were analyzed.Logistic regression was used to analyze the related factors affecting the risk of COPD.Results:The rs679620 locus of MMP-3 gene showed G→A gene mutation,the rs11568818 locus of MMP-7 gene showed A→G gene mutation,the-1082 locus of MMP-12 gene showed A→G gene mutation,the distributions of genotypes conformed to Hardy-Weinberg law.There was no significant difference in the frequency distribution of G and A alleles of MMP-3 gene between COPD group and control group(P>0.0167).The frequency of G and A alleles of MMP-7 gene was significantly higher than that of control group(P<0.0167).The frequency of G and A allele mutations in MMP-12 gene was significantly higher than that in control group(P<0.0167).the mutation frequencies of MMP-3 A,MMP-7 G,and MMP-12 G alleles in COPD group were significantly higher than those in control group(P<0.05);smoking index,bronchial wall thickness and bronchial wall grade of COPD patients with MMP-3 gene A mutation,MMP-7 gene G mutation and MMP-12 G mutation gene were significantly higher than those of GG ge
