某院全肠外营养支持患者营养风险筛查、全肠外营养处方合理性及营养指标变化回顾性分析

摘　　要：目的回顾性分析我院肠外营养支持患者营养风险筛查,处方合理性及营养指标变化。方法调取我院2017年5~11月期间,所有使用全肠外营养(Total parenteral nutrition,TPN)支持治疗≥3d的患者,对患者营养风险筛查2002(Nutritional Risk Screening 2002,NRS2002)使用情况,TPN处方合理性及营养支持后营养指标的变化进行统计分析。结果共纳入85例患者,82例(96.47%)患者使用NRS2002进行营养风险筛查,78(91.76%)例患者评分≥3;TPN处方中,氨基酸浓度<2.5%有8例(7.4%),糖脂比<1∶1有28例(32.94%),热氮比<100∶1的有11例(12.9%),>200∶1的有8例(9.4%),丙氨酰谷氨酰胺质量占总氨基酸百分比>20%有8例(9.41%);白蛋白、前白蛋白及淋巴细胞绝对值较营养支持前均明显增加(P<0.05)。结论我院全肠外营养支持患者NRS2002筛查及TPN处方合理性仍存在一些问题,但对患者营养指标仍有一定改善作用。OBJECTIVE Retrospective analysis on the nutritional risk screening,the rationality of prescription and the change of nutritional parameter of patients with nutrition support in a hospital.METHODS Patients with Total parenteral nutrition(TPN)support≥3d,from May to November 2017.Nutritional Risk Screening 2002(NRS2002)usage,TPN prescription rationality and the changes of the nutrition parameter were analyzed.RESULTS 85 patients were included,82 cases(96.47%)were used nutritional risk screen 2002,and 78 cases(91.76%)were given a score more than 3,8 cases(7.4%)amino acid concentration<2.5%,28 cases(32.94%)the ratio of glucose/lipid ratio were less than 1∶1,11 cases(12.9%)ratio of non-protein energy to nitrogen ratio were less than 100∶1,8 cases(9.4%)were more than 200∶1,8 cases(9.41%)propylene ammonia acyl glutamine quality percentage of total amino acid>20%.The values of albumin,prealbumin,and absolute lymphocytes increased sig nificantly compared with before nutritional support(P<0.05).CONCLUSION There are still some problems with NRS2002 screening and the rationality of TPN prescription in patients with total parenteral nutrition support in our hospital,but there are still some improvement effects on the nutritional parameter of patients.
