摘 要:目的:血清和肩关节滑液miR-21与肩袖损伤严重程度的相关性研究。方法:选择来自我科122例诊断为肩袖损伤的患者及116例同期在我院进行健康体检的人群,取两组人群的血清标本和肩袖损伤患者的肩关节滑液进行总RNA提取,并进行miR-21的实时荧光定量PCR检测,分析血清和肩关节滑液miR-21的相对表达量,同时用ELISA法测定IL-1β和IL-6的表达。将肩袖损伤患者分为肩袖撕裂和肩袖未撕裂组,肩袖撕裂组根据严重程度分为巨大全层撕裂组(MFTT)、非巨大全层撕裂组(NMFTT)、部分撕裂组(PFTT),采用VAS评分和Constant-Murley功能评分对患者肩关节症状功能进行评定,探讨miR-21与撕裂严重程度及症状功能严重程度的相关性。结果:血清miR-21在肩袖损伤患者与健康对照组中无显著性差异,但关节滑液miR-21在肩袖撕裂患者关节滑液的表达量显著高于正常对照组,miR-21表达在MFTT患者关节滑液的表达显著高于NMFTT组,在NMFTT患者关节滑液的表达显著高于PFTT组,miR-21的表达与肩袖损伤严重程度呈正相关性。此外,miR-21表达与VAS评分和Constant-Murley功能评分均呈正相关性,与炎症因子IL-6、IL-1β的表达亦呈正相关性。结论:关节滑液miR-21与肩袖损伤严重程度具有高度相关性,表明局部miR-21的上调促进了肩袖损伤的加重,抑制miR-21表达有可能减轻肩袖损伤及防止肩袖撕裂。Objective: To explore serum and shoulder miR-21 expression with disease severity of rotate cuff injury. Methods: A total of 122 patients diagnosed with rotate cuff injury were recruited in our study. Meanwhile, 116 healthy individuals were also enrolled during their regular body check.Total RNA in both serum and synovial fluid were extract for RT-PCR of miR-21. The rotator cuff injury was classified into Non-tear massive fullthickness tear(MFTT), non-massive full-thickness tear(NMFTT) and partial-thickness tear(PFTT). The symptomatic severity was determined using VAS scores and Constant-Murley scores. Results: The serum miR-21 levels were not significantly different among patients and healthy controls. SF miR-21 levels were higher in PFTT patients compared with non-tear patents. NMFTT patients showed significantly elevated levels of miR-21 in SF compared with those with PFTT. MFTT patients showed significantly elevated levels of miR-21 in SF compared with those with NMFTT. miR-21 levels in SF were positively related the injury degree. In addition, SF miR-21 levels were significantly related to the VAS scores and negatively with Constant-Murley scores. Last, SF miR-21 levels were significantly correlated with SF IL-1β and IL-6. Conclusions: miR-21 in synovial fluid is highly correlated with the severity of rotator cuff injury, which indicates that the up-regulation of local miR-21 promotes the progression of rotator cuff injury, and inhibiting the expression of miR-21 may reduce rotator cuff injury and prevent rotator cuff tear.
