摘　　要：目的研究基质金属蛋白酶-2(MMP-2)、透明质酸(HA)、层粘连蛋白(LN)和Ⅳ型胶原(IV-C)在盆底功能障碍患者血清中的含量变化及其意义;MMP-2在盆底功能障碍患者阴道组织中的表达。方法采用ELISA法检测盆底功能障碍患者绝经前15例和绝经后30例血清MMP-2、HA、LN和IV-C水平,并与无盆底功能障碍患者40例进行比较;用免疫组化法检测盆底功能障碍患者绝经前10例和绝经后20例患者阴道组织中MMP-2的表达,并与正常对照组15例进行比较。结果与正常对照组比较,绝经前和绝经后盆底功能障碍患者血清MMP-2、HA、LN和IV-C水平明显增加(P<0.05);与绝经前比较,绝经后血清MMP-2、LN和IV-C增加更明显(P<0.05);与正常对照组比较,绝经前和绝经后阴道组织中MMP-2的表达明显升高(P<0.05);与绝经前比较,绝经后MMP-2的表达更加明显。结论MMP-2及HA、LN和IV-C在盆底功能障碍患者盆腔脏器脱垂过程中可能起重要作用。Objective To investigate the changes and significance of matrix metalloproteinase-2(MMP-2)and hyaluronic acid(HA),laminin(LN)and Collagen Type IV(IV-C)in patients with pelvic floor dysfunction and MMP-2 Expression in vaginal tissue of patients with pelvic floor dysfunction.Methods The levels of Serum MMP-2,HA,LN and IV-C in 15 premenopausal and 30 postmenopausal patients were measured by sandwich enzyme-linked immunosorbent assay and compared with 40 healthy counterparts.The expression of MMP-2 in vaginal tissues of 10 premenopausal and 20 postmenopausal patients with pelvic floor dysfunction was detected by immunohistochemistry and compared with 15 normal control groups.Results Compared with the results of normal control groups,the levels of serum MMP-2,HA,LN and IV-C levels were significantly increased in premenopausal and postmenopausal patients with pelvic floor dysfunction(P<0.05).Compared with the premenopausal levels,the levels of postmenopausal serum MMP2,LN and IV-C were increased more obviously(P<0.05).Compared with the results of normal control groups,the expression of MMP-2 was significantly increased in premenopausal and postmenopausal vaginal tissues(P<0.05).Compared with premenopausal cases,the expression of postmenopausal MMP-2 was more obvious.Conclusion MMP-2 and HA,LN and IV-C may play an important role in the process of pelvic organ prolapse in patients with pelvic floor dysfunction.
